SUPPLEMENTARY INFORMATION Report No.: 50-302/79-087/0lT-5 Facility: Crystal River Unit #3 Report Date: 25 February 1980 Occurrence Date: 18 February 1980 Identification of Occurrence: Inadequate design of piping supports reportable under Technical Specification 6.9.1.8.1.

Conditions Prior to Occurrence: Mode 1 power operation (100%)

Description of Occurrence: At 1700 as a result of a re-analysis of pipe supports, conducted in accordance with IE Bulletin- 79-14, it was discovered that thirty (30) supports would require additional modification. The pipe support adequacy criterion fell into two (2) categories, one of which was the requirements of IE Bulletin-70-02 (concrete inserts);

and the other was basic pipe support design. Of the thirty (30) supports requiring modification, eleven (11) supports are of the non-79-02 category. These include:

EFH 64, 78, 84, 88, 510 MSH 547 MUH 602A, 623, 710, 910 SWH 416 An additional fifteen (15) supports ur the non-79-02 category require minor field adjustments. These include:

EFH 48, 67, 69, 71, 72, 73, 104, 151, 152 LRR 4 MSH 555, 559 MUH 604, 616, 645 There are four (4) suppor.ts requiring modification due to the require-ments of 79-02, These are:

DHH 617 LRH 26, 34A MUH 799 Two other supports (EFH-135 and SFH-692) have safety factors between four (4) and eight (8) and required prying analysis. It was later determined (0800 on February 1980) that spent fuel hanger SFH-692 had a safety factor in the East-West direction of 1.76, 3.12 in the North-South direction, and 14.12 vertically. EFH-135 was found acceptable.

Designation of Apparent Cause: The cause of this event is attributed to inadequate design.

Analysis of Occurrence: No effect upon the plant or general public. Plant operations will not be affected by these supports in their present condition. For double OBE loading of the non-79-02 category, eleven (11) supports re-quire modification so as not to exceed allowable stress limits. Ten (10) of these supports have stress levels below yield values and will perform their intended function. The other support is overstressed due in large part to the thermal load. This support has functioned satis-factorily under thermal loadings since commercial operation (March 1977).

Since the seismic loading is a small percentage of total loadings, this support is expected to perform its intended function during a seismic event.

Supplementary In fo rmat ion 50-302/79-087/OlT-5 Analysis of Occurrence: Another fifteen (15) supports of the non-79-02 category (Cont'd) require minor field adjustments.

Those four (4) supports falling into the 79-02 category that have been identifiea as having a safety factor between two (2) and four (4) have at least three (3) supports upstream and downstream of the affected hanger.

Corrective Action: Modifications 80-2-72 and 79-6-86 have been initiated to repair the affected supports to meet the requirements specified in the Bulletin.

Rework is scheduled to be performed during the next planned outage.

SF11-692 was repaired at 0200 on 23 February 1980 raising the East-West safety factor to 9.2. A modification has been initiated to raise the safety factor in the North-South direction to 8.9. This repair will be completed prior to returning operational af ter the current outage.
