TOLEDO EDISON COMPA';Y DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SPECIAL REPORT 1l DATE OF EVENT: January 30, 1979 FACILITY: Davis-Besse Unit 1 Photoelectric Detector DS 8605-E did not pass IDENTIFICATION OF OCCURRENCE:

Surveillance Test ST 5016.04 i

i The unit was in Mode 1 with Power (MWI) = 2550, Conditions ?rior to Occurrence: I and Load (Gross MWE) = 825.

Description of Occurrence: While conducting Surveillance Test ST 5016.04, " Detector Channel Functional and Supervisory Circuits Checks", it was determined that Photo-electric Smoke Detector DS 8605-E in the Emergency Diesel Generator This lead 1-1 area to DS 8605-E was being g not functional and a test deficiency was p epared.

declared inoperable at 1600 hours0.0185 days
0.444 hours
0.00265 weeks
6.088e-4 months
on Janus v 30, 1979, which placed the unit in the Action Statement of Technical Specification 3.3.3.8, which specifies the'ainimum fire detection instrumentation and requires with one or more detectors inoperable 1

that within one hour, a fire watch patrol be esAablished fire watchand the was patrol instrument (s) established restored to operable status within 14 days.

at that time. .

Designation of Apparent Cause of Occurrence: The failure of Photoelectric Detector DS 8605-E was the cause of the occurrence.

Analysis of Occurrence: The f our other area pnotoelectric detectors in this open s

rec _ are operable, and a fire watch patrol was naintained in this area.

1l Corrective Action: A fire watch patrol was maintained in the area, and a new detec-1 tor was ordered. The new detector was installed under Maintenance Work Order I6C 013-005-79 and retested satisfactorily at 1529 hours0.0177 days
0.425 hours
0.00253 weeks
5.817845e-4 months
on February 28, 1979, removing the unit from the Action Statement of Technical Specification 3.3.3.8.

Failure Data:

There have been no p;eviously reported failures of photoelectric detectors.
