5. Identification of Occurrence:

Diesel generators were not tested as required by Technical Specifications due to non-compliance to operating procedures.

6. Conditions Prior to Occurrence:

7. Description of Occurrence:

An instrument technician installed a replacement speed switch on the "A" Energency Diesel Generator without having his Job Order approved by the shift supervisor as required. With the shift supervisor unaware of the "A" diesel being out of service, neither the "A" nor "B" diesel were tested.

9. Analysis of Occurrence:

The instrument technician forgot to obtain the shift supervisor's approval before beginning vorP., causing a violation of Technical Specification 3.8.1.1.(b). There was no danger to the health and safety of the public.

The technician was re-instructed on the processing of Job Orders and the "A" diesel generator was successfully tested.
