On February 1,1979, it was discovered that the litits of LCO NR 1.3 had been exceeded twice during 1978. Subsequently it was determined the limits of LCO NR 1.1 had been exceeded during January,1979. These events are reportable per Fort S t. Vrain Technical Specification AC 7.5.2(b)2.

EVENT DES CRIPTION :

LCO NR 1. 3 The total quantity of cooling tower blowdown, as determined by the difference in blowdown integrator readings taken at 0000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> one day to 0000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> the next day, exceeded the limit of LCO NR 1. 3 which is 3. 75 million gallons per day (MCD). On June 14, 1978, the blowdown was 4.449 MGD. On Decerber 12, 19 78, the blowdown was 3. 84 MGD.

LCO NF 1 On January 16, 1979, the weekly effluent analysis showed phosphorous to se 15.4 ppm which is greater than the LCO limit of 14 ppe. The follow up sa=ples continued until January 22, 1979. On January 21, 1979, the value was 14.5 ppm and on January 22, 1979, the value was 16.5 ppm.

These conditions were discovered during an audit of surveillance records.

The surveillance form did not clearly call out what deficiencies were to be reported and chemistry personnel did not conplete the surveillance form cor-rectly.

CAUS E DESCRIPTION:

The cause of the excessive blowdown rates are undetermined, but these appear to be unrelated incidents as the checistry reports for both days f ail to in-dicate any need for excessive discharges and all chemical concentrations were within acceptable limits.

The source of the high phosphorous levels are undetermined, but appear to have occurred outside the main plant buildings. The effluent sa=ple is taken from the Goosequill Ditch and several possible sources of the phosphorous exist downstream of the main plant water discharges. This nakes it difficult to deter =ine where the phosphorous originated. The chemical samples were taken and analyzed properly with follow up sampling continuing on a daily basia, until the phosphorous level was lower than action level II limits.

Howeve r , the analysis results were not properly recorded and the surveillance for=s were not properly co=pleted.

The cause for not reporting the incidents when they occurred was incorrect completion of the surveillance form by chemistry personnel. This could, in part, be attributed to the surveillance form not being clear on what should be reported as a deficiency and the actual LCO limit not appearing on one of the data sheets.

CORRECTIVE ACTION :

The excessive blowdown rates were corrected the following day.

The effluent phosphorous levels decreased to less than action level II 21=its.

The surveillance form was revised to show the LCO -limits clearly and chemistry personnel were informed of the correct method for making deficiencies on the surveillance form.

No further corrective action is anticipated or required.
