SUPPLEMENTARY INFORMATION Report No.: 50-302/79-087/01T-3 Facility: Crystal River Unit #3 Report Date: 2 November 1979 Occurrence Date: 26 October 1979 Identification of Occurrence:

Pipe stress at a 3 x 2 reducer on the outlet of RCP-1A motor thrust bearing cooler exceeds the FSAR required allowable stress.

Canditions Prior to Occurrence:

Mode 1 power operation (75%)

Description of Occurrence:

At 1412 as a result of a verification of computer analysis of seismic restraints, it was discovered that the pipe stress at a 3 x 2 reducer en the outlet of RCP-1A notor thrust bear-ing cooler exceeds the FSAR allowable stress. Further in-vestigation revealed that the pipe stress at the reducer does not exceed current accepted Code requirements.

Designation of Apparent Cause:

The cause of this event is attributed to inadequate design.

Analysis of Occurrence: ,

No effect upon the plant or general public. Automatic isolation valves are available to isolate the affected section of this line from the remainder of the SW system.

Corrective Action:

. Modification 79-10-85 has been initiated to add a restraint to reduce the pipe stress at the 3 x 2 reducer within the FSAR requirements. Repairs will be completed during the next plant outage.

Failure Data:

This is the fifth occurrence of this type reported. Reference LER's 79-061/0lT-0 and 79-087/0lT-0, 1 and 2.
