In response to IE Bulletins 79-02 R1 and 79-02 R1, Supplement 1, field inspections of anchors on various piping systems (2151 anchors inspected to date) have revealed 163 deviations. On non-safety-related piping,18 bolt anchors were pull tested, two slipped,and two pulled out.

Conditions ' Prior to Occurrenec:

Plant in normal operation. Unit 1 - 1035 We, unit 2 - 1095 We, unit 3 - refueling outage.

Anchorages were shortened due to rebar interference.

Ana.l r.is of Occurrence:TVA's Division of Engineering Design analysis indicates all l

anchorages with deviations to be adequate with safety factors greater than 2.

The evaluation of the results of the ongoing inspection program continues. Subsequent to the four initial pull-test failures, 52 inchors were pull-tested on non-safety-related piping and 40 anchors tested on safety-related piping with all anchors passing at proof loads.

Corrt.ctive Action:

A repair program has been initiated by field forces to correct improperly installed anchors.
