identif" sticn and Descr;ption of Occurrence: During inspections in accordance with IE Bulletin 79-14, PHRSW restraint R-8 was found to be inoperable in that of the four bolts required, the nuts on two through-bolts had fallen off and two through-bolts were missing.

Conditions Prior to Occurrence:

Unit 1 at 1045 MWe.

Unit 2 at 1095 MWe.

Unit 3 at 0 MWe - Maintenance Outage Action specified in the Technical Specificcticn Surveillance Requirements met due to tnoperable equioment. Describe.

None App <trent Cause of Occurrence:

Pipe vibration and/or improper installation during construction apparently caused the two nuts to loosen and fall off. The missing through bolts were apparently not installed during constructirn.

U bhia Cor' ective Action :

Restraint R-8 has been repaired to design specifications. EN DES performed an evaluation and supplied a list of 41 similar restraints in units 1, 2 and 3 to be inspected. Of these,31 have been inspected and of those inspected R-5, R-18, R-19,and R-22 on the RHRSW system. required repair work. A followup report will be issued at a later date.
