o Loperation, Low Pressure Safety Injection Discharoe Header valve i o . I 2SI-5091-3 was discovered to be in the closed position. The redundant I r8 FO 1 flow path, thru 2CV-5091, was operational; therefore, full Low Pressure i FETin Injection flow would have been available if required. There have been i o , i no similar occurrences. This occurrence is reportable per T.S.6.9.1.9.c.
