,P During power operation, an evolution was performed.'o. place the Unit No. 0 spent.

. fuel pi't (SFP) purification.demineralizer in service.

Because of'.incomplete communication and. faQ.ure to, follow,a pxocedure, 'it was not recognized, that this shazed. deminexalizer was already in, service in the refueling water. storage tank (EST) puxification mode..

Consequently, when the correct va1ve alignment was made to establish the spent, fuel -pit.puxification flow path, an unanticipated flow.path was inadveztently'zovvided which allowed. the refueling, water purification pump to transfer.,water fxom, the refueling water storage tarik to the spent fuel p'it.. Prior to discovexy and, coxxection of this improper configuration, approximately 8,000 gallons of water weze transferred..

This. resulted. in (1) a reduction of the. volume in the refueling, water storage tank to appxoxi.mately 317,000 gallons (TS minimum: 320,000'allons) and. (2) overflow of about 3,000 gallons;, fxom the spent fuel pit,.

Upon.discovery of this occurrence, a Unit No. 4 shutdown was initiated and pursued. for 45-minutes until the, required volume was zeestablished.
