Dear Sir:
Entergy Nuclear Operations, Inc. (ENO) is reporting, pursuant to Technical Specification 5.6.8, the Steam Generator (SG) tubes plugged following the Refuel Outage 12 Inservice Inspection (0SI) of SG at Indian Point 3 Nuclear Power Plant. The ISI, completed April 12, 2003, identified twelve tubes that were plugged. The SG Tube Inspection Report will provide additional details on the extent of inspection, each indication of an imperfection, and plugging/repair. The following table identifies the indication (PVN is permeability variation and VOL is volumetric wear) and, where applicable, the through wall (TW) wear for the twelve tubes plugged:
