On 10/22/86 during an investigation into the cause of LER 86-022 it was determined that unauthorized access may have been gained into th

Effect of Event:
Unauthorized access into th which is a Vital Area, may have been allowed.
That this unauthorized access actually occurred is still being investigated.
In a preliminary review o t~ appears that unauthorized persons entered the area. Howi:,er, upon interrogation of those individuals it was determined that they had not.

MDescri Description of Apparent Cause:
The cause of this problem is still under investigation, however it appears to be card reader malfunction.
Corrective Action
The malfunctioning card reader was replaced.
Th investi tion we have discovered that one individual did enter t ithout authorization.
This occurred on October 16 at 0942
