Investigation revealed that this was l

one in error when a computer technician utilized the wrong procedure when reprogramming a set of card readers for the l'nf t 2 Outage.

Khen this did not result in proper logging at Doo investigation and corrective acti resulted in revising the technique and procedure. The fact that Doo d been changed by the original action was ove rlooked. As a result any Investigation reveals that this condition existed from 9/86 to 10/18/86.

The procedure to set up specific card readers for logging purposes is being revised.

In order to verify database changes a hardware modification was made that ad to the console to rammer terminals at the CAS and SAS.

1)

All defective keys that are being held for reorder will be in eted to ensure we have theh If we do not have th n it will not be reordered, but will be classified as and the number will never be reissued.

2)

Procedures that detail the handling of defective keys will be revised to reflect the above change.

4.

An in-depth review of configuration control on the Units 1, 2, and 3 security system will be performed.

Any necessary corrective actione. will be taken and we will implement necessary procedures to ensure complim e.

The review of configuration control is on-going.

It is anticipated that this will be completeo by 2/28/87.
