between elevations 122' and 100' in the Auxiliary Building, was discovered open by a Fire Department roving patrol.

No fire watch l

was posted as required by Technical Specification Action Statement 3.7.11.a.

The root cause of this event is personnel error due to inadequate communication of the requirement to treat the subject hatch as a fire barrier which requires an Impairment Permit prior to l

opening it.

Upon discovery of the open hatch a roving fire watch was established to maintain surveillance of the impairment until the hatch was closed.

To prevent future recurrence of this event a letter to all applicable personnel has been issued specifying floor hatches in the Auxiliary Building (except those with specific exemption) are considered fire barriers and opening them is an impairment requiring compliance with Administrative Procedure AP-25,

" Fire Protection Program".

Also, the hatches which are fire barriers, will be uniquely identified.

Technical Specification 3.7.11 states:

All fire penetrations (including cable penetration barriers, I

fire doors and fire dampers), in fire zone boundaries, I

protecting safety related areas shall be OPERABLE.

i Action Statement 3.7.11.a states:

With one (1) or more of the above required fire barrier penetrations inoperable, within one (1) hour either establish a i

continuous fire watch on at least one (1) side of the affected penetration, or verify the OPERABILITY of fire detectors on at least one (1) side of the inoperable fire barrier and establish an hourly fire watch patrol.

Restore the inoperable fire barrier penetration (s) to OPERABLE status within seven (7) days or, in lieu of any other report required by Specification 6.9.1, prepare and submit a Special Report to the Commission pursuant

DESCBIETIQU_QE.QCCUBBEUGE1 to Specification 6.9.2 within the next thirty (38) days outlining the action taken, the cause of the inoperable penetration (s) and plans and schedule for restoring the fire barrier penetration (s) to OPERABLE status.

6EE6BEUT_C&USE 0E_0CCUBBENCEL The Salem Generating Station Fire Hazards ' Analysis, specifies each elevation of the. Auxiliary Building as a fire zone with the floors as boundaries.

Removal of a hatch is therefore considered an impairment of the floor fire zone boundary and must be controlled in accordance with Technical Specification 3.7.11.

This equipment hatch is part of the floor fire barrier between the 100' and 122' elevation of the Auxiliary Building..Upon removal, the barrier is considered impaired.

The equipment hatch _is unmarked and has been historically used to facilitate large equipment movement between elevations (as was the case which resulted in this event).

The equipment hatch had. remained open for approximately twelve (12)-

l hours (in which time the equipment was moved) prior.to its discovery l

by a Fire Department roving patrol.

A Fire Protection Impairment Permit was not ' initiated nor was a fire watch (roving or permanent) established in accordance with Technical Specification 3.7.11.

The fire detectors were operable during the entire period in which the equipment batch was impaired.

The root cause of this event is personnel error due to inadequate communication of the requirement to treat the subject equipment hatch as a fire barrier which requires an Impairment Permit for opening it.

4B& LYSIS _QE_QCCUBBENCEL The functional integrity of the penetration fire barriers ensures that fires will be confined or adequately retarded from spreading to adjacent portions of the f acility.

This design feature minimizes the possibility of a single fire rapidly involving several areas of the facility prior to detection and extinguishment.

Penetration fire barriers are passive elements in the facility fire protection prograc and are subject to periodic impairment.

Technical Specification Action Statement 3.7.11.a provides conditions under which these barriers may be impaired.

Since the Action Statement requirements cannot be shown to have been met (by posting of a roving or permanent fire watch) this event is reportable in accordance with the Code of Federal Regulations,10CFR 59.73(a) (2) (1) (B).

There are no components / equipment on either side of the equipment hatch in question which require protection from fire to assure the safe shutdown of the plant.

Therefore, this occurrence involved no undue j

risk to the health or safety of the public.
