PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE -- PNO-I-09-005A This preliminary notification constitutes EARLY notice of events of POSSIBLE safety or public interest significance. Some of the information may not yet be fully verified or evaluated by the Region Staff.

UPDATED - OYSTER CREEK REACTOR TRIP AND UNUSUAL EVENT DUE TO A LOSS OF OFFSITE POWER AND SHUTDOWN GREATER THAN 72 HOURS This preliminary notification updates information in PNO-I-09-005, which documented a July 12 loss of offsite power, resultant reactor scram and Unusual Event. At the time of the last notification, offsite power had been restored, the Unusual Event had been terminated, and the plant had achieved cold shutdown.
The resident and regional inspection staff monitored the licensee's actions to address equipment issues prior to plant restart. The Q121 breaker, which failed to open, is being maintained open until the cause of the failure is identified and corrected by the electrical grid operator. The open Q121 breaker has no adverse effect on plant operation. The delayed loading of the #1 EDG was determined to be caused by a faulty auto-sync module, which has been replaced and successfully tested. The erroneous 'B' isolation condenser level indication was determined to be caused by foreign material in the sensing line and the licensee has developed a program to flush and monitor these lines to prevent recurrence. The resident staff and regional specialists inspected these issues to verify restart readiness.
On July 16, Exelon took the reactor critical at 12:12 a.m. and placed the turbine in service and connected the plant to the electrical grid at 3:13 p.m. Oyster Creek is currently at 90-percent power as Exelon resolves an issue with one of the four circulating water pumps failing to start. The Resident Inspectors observed portions of the plant startup and continue to follow the licensee's corrective maintenance activities.
The Region dispatched a Special Inspection Team (SIT) on July 16 to review and assess the equipment issues and station response to the event (see Press Release dated July 16, 2009, No. I-09-035).
State and local officials have been informed. Region I Public Affairs is prepared to respond to media inquiries.
