We have reviewed our initial response to IE Inspection Report 82-29 .

(L-82-469, dated October 29,1982) and the comments of the resident in-spector presented in IE Inspection Report 82-37. A revised report to the finding in Inspection P,eport 82-29 is attached.

Contrary to the above, steps 4.5, 8.5.2 arid 8.6 of Admi ni strative Procedure 0103.4, "In Plant Equipment Clearance Orders" were not fully implemented for equipment clearance order (ECO) 7-155 in effect from 7/19/82 to 7/23/82. A danger tag was improperly pl a ced , the ECO f o rm wa s not c om pl e ted , and the clearance wa s not properly removed from valve MOV-878B in tne safety injection system.

RESPONSE

(1-1) FPL concurs wi th the finding. .

(1-2) The reason for this finding was that tne operator i nad ve rtently did not specify on the clearance orcer that the brea ker fo r MOV-4-878B sh oul d be t agged and tne tag intended fo r the MOV swi tch on Ve r t i cal Pa nel B was inadvertently hung on the manual operator of tne MOV instead.

(1-3) As corrective action, the da nger tag wa s removed from MOV-8788.

(1-4) The operator wa s counseled on the need for proper c om pl e t i o n of the equipment clearance order and com pli a nce wi th pl a nt procedures. Tne Admi ni strative Procedure 0103.4, In-Plant Clearance Orders, nas been r ev i sed to better describe where clearances are placed and to change the provisions for tempora ry clearance releases.

(1-5) Full compli ance wa s achi eved on September 30, 1982.
