11-9-2017 | On September 21, 2017 at 0800 PDT, Columbia Generating Station declared number 2 Diesel Generator inoperable for maintenance and entered Technical Specification Action Statement (TSAS) 3.8.1, Condition B for One Required Diesel Generator inoperable. At 0821 PDT and again at 1537 PDT Surveillance Requirement 3.8.1.1 was performed per action statement B.1. At 2315 PDT it was noted by operations that a page was missing from the surveillance procedure for both occurrences and determined that the action statement was not met within the required action time. Operations entered TSAS 3.8.1, Condition F for Required Action and associated Completion Time of Condition B not being met. AT 2321 PDT SR 3.8.1.1 was performed satisfactorily and Condition F was exited.
The apparent cause was a failure to page check the surveillance procedure prior to use. Initiatives have been developed to reinforce existing standards and improve human performance.

Columbia Generating Station 05000- 397 2017 - 006 - 00
PLANT CONDITIONS
At the time of the event, Columbia Generating Station (Columbia) was in Mode 1 operating at 100% reactor power.
Number 2 Diesel Generator [EK] [DG] (DG2) was out of service.
EVENT DESCRIPTION
On September 21, 2017 at 0800 PDT, Columbia declared DG2 inoperable for maintenance and entered Technical Specification Action Statement (TSAS) 3.8.1, Condition B for One Required Diesel Generator inoperable. At 0821 PDT and again at 1537 PDT Surveillance Requirement (SR) 3.8.1.1 was performed to satisfy TSAS 3.8.1, Condition B action statement B.1. At 2315 PDT it was noted by operations that a page was missing from the surveillance procedure used to satisfy SR 3.8.1.1. For both occurrences the action statement was not met within the required action time. Operations entered TSAS 3.8.1, Condition F for Required Action and Associated Completion Time of Condition B not being met. AT 2321 PDT SR 3.8.1.1 was performed satisfactorily and TSAS 3.8.1 Condition F was exited.
CAUSE
The direct cause was due to pages sticking together when the surveillance procedure was being copied for use. The apparent cause was the failure to perform a page check of the procedure prior to use. The contributing cause was the reviewing supervisors failure to verify completeness and accuracy of the surveillance once it was completed.
EXTENT OF CONDITION
A review of recently performed on demand procedures was conducted and no instances of missing pages was noted.
COMPLETED CORRECTIVE ACTIONS
Missing pages were added, the procedure was performed, and SR 3.8.1.1 was completed satisfactorily.
Individual contributor culpability was evaluated and initiatives were developed to reinforce existing standards for performing page checks and to improve human performance.

ASSESSMENT OF SAFETY CONSEQUENCES
The off site power alignment surveillance is performed to ensure the electrical system will function as designed when required. The missing page of the surveillance contains steps that must be performed to satisfy a TS surveillance requirement. Since these steps were not performed within the required time frame, the station did not meet the required action of the Limiting Condition for Operation (LCO) to perform SR 3.8.1.1. Compliance with LCOs is necessary to ensure comments regarding burden estimate to the Information Services Branch (T-2 F43), U.S. Nuclear Regulatory Commission, Washington, DC 20555-0001, or by e-mail to used to impose an information collection does not display a currently valid OMB control number, the NRC may not conduct or sponsor, and a person is not required to respond to, the information collection.
Columbia Generating Station 05000- 397 2017 - 006 - 00 the plant is operated above minimum requirements for ensuring safe operation of the facility. There were no actual safety consequences as a result of this event as it was administrative in nature and the required breaker alignment was verified to have been correct for the affected period.
PREVIOUS OCCURENCES
A review of Columbia LERs for the past three years did not identify similar events where human performance issues caused the station to not meet TSASs.
