Failure to Establish Heat Exchanger Inspection Procedures Appropriate for the Circumstances

Description

The inspectors identified a finding of very low safety significance (Green) and an associated NCV of 10 CFR Part 50, Appendix B, Criterion V, Instructions Procedures, and Drawings, for the licensees failure to ensure that activities affecting quality were prescribed by documented procedures of a type appropriate to the circumstances. Specifically, the licensee failed to ensure that procedure ERAA3401002 appropriately accounted for partially blocked HX tubes identified during HX inspections.
