Title | Failure to Document a Degraded Condition on the AFPT 1 Outboard Bearing |
Description | The inspectors identified a finding of very low safety significance for the licensees failure to document a degraded condition of a safety-related system in the corrective action program (CAP), as required by licensee procedure, NOPLP2001. Specifically, during planned maintenance on auxiliary feedwater pump turbine (AFPT) 1, the licensee identified scoring on the outboard turbine bearing and failed to generate a condition report detailing the issue. The licensee entered this issue into the CAP as condition report (CR) 201712487 for evaluation. The inspectors determined the performance deficiency was more than minor because if left uncorrected it had the potential to lead to a more significant safety concern. Specifically, the failure to document a degraded condition in the CAP did not allow the organization to properly assess the issue. Therefore, the underlying cause may not have been appropriately addressed. Using IMC 0609, Attachment 4, Initial Characterization of Findings, issued October 7, 2016, and Appendix G, Attachment 1, Shutdown Operations Significance Determination Process Phase 1 Initial Screening and Characterization of Findings, issued May 9, 2014, the inspectors determined the finding to be of very low safety significance (Green) because the inspectors answered no to all questions in Exhibit 3 of Appendix G, Attachment 1. The inspectors determined this finding affected the cross-cutting aspect of identification in the area of Problem Identification and Resolution, where the organization implements a corrective action program with a low threshold for identifying issues and individuals identify issues completely, accurately, and in a timely manner in accordance with the program.
