| Description | A self-revealing Greennon-cited violation (NCV) of Technical Specification (TS)5.4.1.a, was identified for the licensees failure to have adequate written procedures for assuring proper configuration control in areas affected by maintenance or plant modifications. Specifically, the licensee failed to detect and correct a disconnected lead from contact C1 on 1-SW-62-1SWEB03. This directly led to the failure of the 1B service water (SW) pump to shed from the 1J emergency bus during performance of maintenance procedure 1-PT-83.2 on March 11, 2018. |
