| Title | Steam Dump Closure Caused by Human Error |
| Description | On May 10, 2018, a Green self-revealed finding and associated Non-Cited Violation occurred when licensee personnel caused the Unit 2 steam dump valves to the condenser to close. Specifically, when tuning the controller for the steam dump valves, licensee personnel left the controller in automatic, resulting in the closure of all the steam dump valves. This caused both the steam generator power operated relief valves and a steam generator safety valve to lift. |
