| Title | Licensee-Identified Violation |
| Description | The following violation of very low significance (Green) was identified by the licensee and is a violation of NRC requirements which meets the criteria of the NRC Enforcement Policy for being dispositioned as an NCV. Title 10 CFR 50, Appendix B, Criterion V, Instructions, Procedures, and Drawings states, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings appropriate to the circumstances and shall be accomplished in accordance with those instructions, procedures, and drawings. Equipment tagging is a safety related process implemented by procedure 12OHP2110CPS001, Clearance Permit System. Contrary to 12OHP2110CPS001 step 4.4.3, which directs operators to comply with the tagout on the Unit 2 East Motor Driven AFW pump room cooler, the operators mistakenly secured and tagged the Unit 1 East Motor Driven AFW pump room cooler instead. This rendered the Unit 1 East Motor Driven AFW pump inoperable. The violation occurred at 0219 on September 6, 2017, and concluded at 0623 the same day after the error was realized and corrected. The licensee entered the issue into their CAP as AR20178509. The finding screened to Green because there was no loss of system function, nor loss of a train for greater than the Technical Specification allowed outage time. |
