| Title | Failure to Follow Maintenance Procedures for Assembling Unit 3 HPCI Room Cooler Fan |
| Description | A self-revealing, Green non-cited violation (NCV) of Technical Specification (TS) 5.4, Procedures, was identified for the licensees failure to follow maintenance procedures DMP 570004, LPCI and HPCI Room Cooler Maintenance, and DEP 570004, HPCI Room Cooler Fan Preventive Maintenance, when assembling the Unit 3 HPCI room fan. Specifically, on one occasion when maintenance was performed on the fan, technicians installed the cam locking collar in the opposite direction of the fan shaft rotation, and on the other occasion, technicians tensioned the fan belt to the wrong value and misadjusted the alignment of the shaft sheave. Over time, this improper maintenance caused the inboard and outboard fan bearings to wear on the shaft, causing increased vibrations, and eventually leading to HPCI being declared inoperable to emergently work on the fan |
