| Title | Failure of M1A Transformer Causes an Automatic Load Reject Scram |
| Description | An unresolved item was identified to review Exelons root cause assessment and licensee event report (LER) regarding the failure of the M1A main transformer and subsequent load reject scram to determine whether a performance deficiency existed which contributed to the transformer failure. The inspectors plan to review Exelons evaluation after it is completed, which had not occurred by the end of this inspection period. At 2101, November 28, Oyster Creek experienced a generator trip due to an A-phase and B-phase differential relay actuation, which resulted in a reactor shutdown due to a load reject scram. All safety systems operated as expected during the scram. The grid disturbance report provided by Jersey Central Power & Light, combined with information from the Oyster Creek Digital Protective Relay System, differential voltage and current indication data, and dissolved gas in oil analysis indicated that the fault occurred on the B phase of the M1A Main Power Transformer.
