90-Day Post-Outage Inservice Inspection (ISI) Summary Report In accordance with 10 CFR 50.55a, "Codes and standards," paragraph (g), "Inservice inspection requirements," AmerGen Energy Company (AmerGen), LLC is required to maintain an Inservice Inspection Program in accordance with the American Society of Mechanical Engineers (ASME) Boiler and Pressure Vessel Code. Please find enclosed the Post-Outage (90-Day) ISI Summary Report for Clinton Power Station for examinations and repair/replacement activities performed between May 7, 2002 (i.e., the Spring 2002 refueling outage) and February 27, 2004 (i.e., the end of the most recent refueling outage).
This report is submitted in accordance with the requirements of ASME Section Xl, Article IWA-6200, Paragraph IWA-6230. This refueling outage was the first outage scheduled of the second inspection period of the second inspection interval. The second inspection interval is from January 1, 2000 to December 31, 2009.

Page 1 of 2 SECTION 1 INTRODUCTION AND SCOPE INTRODUCTION:
This Summary Report covers the Inservice Inspection performed at Clinton Power Station (CPS), between May 8, 2002 and February 25, 2004. This is the first inspection performed during the second inspection period of the second ten (1 0)-year interval.
During this Fuel Cycle, CPS experienced four forced outages (CIF37, CIF38, CIF39, and CIF40) and two planned outages(CIM13 and CIR09). All examinations, tests, repairs and replacements were performed in accordance with the CPS Inservice Examination Plan which complies with ASME Section XI, 1989 Edition.
The ASME Section XI examinations, tests and repairs/replacements were witnessed or verified by Authorized Nuclear Inservice Inspector M. Haydon of Hartford Steam Boiler of Connecticut. This summary report contains additional augmented (non-ASME Section XI) inspection results not required to be witnessed or verified by the ANTI.
In accordance with IWA-6220, the name and description of the components examined are listed on the NIS-1 Data Report and its attachments (Section 2). Further component information is obtainable through the reference number, or other information, for the specific item. The manufacturers' names for the components examined are also identified in Item 7 of the NIS-1 Data Report and their addresses are on file at CPS.
Documentation supporting this Summary Report is referenced in the Inservice Examination Plan, implementing procedures, or record files identified in the specific sections of this report.
Scope:
The scope of Inservice Inspection performed during this Fuel Cycle is summarized as follows:
- Preservice Examinations - Preservice examinations were performed on bolting associated with the main steam safety relief valves and various flange boltings.
Also, preservice examinations were performed on two (2) piping welds as a result of modification to the RCIC system during CIR09.
- Scheduled Inservice Examinations - The NIS-I Data Report (Section 2) provides the listing of all examinations completed during CIR09. Conditions noted during ClRO9 and corrective measures taken are described in the NIS-1 Data Report.

- Snubbers - There were 579 snubbers in the 1SI Program required by CPS Operational Requirements Manual (ORM). A representative sample of 59 snubbers (10% of each Type) was selected for functional testing. Two (2) snubbers which were identified in Cl R08 as degraded were also tested for Service Life Monitoring program.
As a result of two (2) snubber failures, additional snubbers were selected for testing.
Overall, there were nine (9) snubbers identified as not meeting the functional test acceptance criteria. A total of one hundred and sixteen (.16) snubbers were tested to meet the CPS ORM requirements. Attachment 2 discusses the details of failure analysis and corrective action taken to address these failures.
CPS implemented 4 modifications (MSFOI2, 341501, 341503, and 341504) eight (8) snubbers (I MS46001 Sj.l MS48003S, I MS48005S, IMS 10602S, I RT28006S, IRT28007S, IRT31004S , and IRT46001S ) were removed permanently from the program. The snubbers population afterthe CIR09 is now 571.
- Component Supports - Thirty one (31) components supports (other than snubbers) and five (5) Class 3 integral attachments were scheduled for visual examination for Cl R09. All were found acceptable.
- Pressure Tests - ASME Code Class l Components - A system leakage test and VT-2 inspection of ASME Code Class I components were successfully performed prior to plant startup.
- Repairs and Replacements - During CIR09 no repairs and replacements were required as a result of scheduled examinations. Also during CIR09, there were several modifications performed. The modifications, as well as repairs and replacements which resulted from routine maintenance, are specifically detailed in Section 6 of this report.

Table I lists scheduled examinations of components for CIRO9. The results column in the table provides further explanation for these examinations. Those items, which were examined, that require no further evaluation are identified as acceptable. Table 2 lists those items added to the Cl R09 scope for various reasons and examined. These items require no further evaluation and are identified as acceptable.
Table 3 lists scheduled visual examinations of component supports. The results column in the table provides further explanation for these examinations/tests. Those items, which were examined/tested, that require no further evaluation are identified as acceptable. The visual examination results of all supports were acceptable.:
Table 4 lists visual examinations and functional testing of snubbers. The results column in the table provides further explanation for these examinations/tests. Those items, which were examined/tested, that require no further evaluation are identified as acceptable. The visual examination results of snubbers are identified as acceptable. For those snubbers, which had unacceptable functional tests, the additional snubbers tested are included in this table and discussed further in attachment 2.
Table 5 lists scheduled pressure tests. The results column in the table provides further explanation for these tests.
Current Interval Status:
Currently CPS is in the Second Period of the Second Interval. CPS had two (2) refueling outages, CIR07 and CIR08 during the First Period. CPS will have two (2) refueling outages, CIRO9 and CIROIO, during the Second Period. CPS possibly will have one (1) refueling outage, CIROI 1,during the Third Period. With the exception of the examinations that may be deferred until the end of the inspection interval, the required examinations in each examination category were completed (Inspection Program B).
During the seventh refueling outage (CIR10) two (2) items were identified as requiring relief from NRC. These items are identified as N7 and RHR-A-2. During the eighth refueling outage (CI R08) ten (10) items were identified as requiring relief from NRC. These items are CH-C-2, RPV-C5, N2F, N2G, N3A, N3C, N4A, N4D, NSA, and N9A.
During the ninth refueling outage (CIROI I) one (I) item was identified as requiring relief from NRC. This item is identified as I-MS-A-7PR-WA.

Steam Dryer - The existing crack on the steam dryer channel #8 to the skirt was examined and determined that it has grown 1 1/8 inches in one cycle. This is not an ASME Section XI item. See attachment 2 for further discussion.
Note 2:
Steam Dryer -- The steam dryer was examined per GE SIL # 644, Supplement 1. During this examination of the dryer's lower guide at zero degrees, it was observed that the lower left horizontal stiffener was found with mechanical deformation. This is not an ASME Section XI item. See attachment 2 for further details.
Note 3:
Steam Separator - During the separator examination of the lower guide at zero degrees, it was observed that the lower left outside corners were found with mechanical deformation. This is not an ASME Section XI item. See attachment 2 for further details.
Note 4:
Core Spray Internal piping - In accordance with BWR VIP-I 8 document, Ultrasonic examinations were performed on the selected welds on Core Spray piping. Indications were recorded on three (3) of these welds (BP2, CP2, and DP2). This is not an ASME Section XI item.

Note 5:
Steam Dryer Support Bracket - Several of the brackets appear to have Contact mark and some do not have any contact marks with the steam dryer. This is not an ASME Section XI item. See attachment 2 for further discussion.
Note 6:
Surveillance Specimen Brackets @ 003 and 177 degrees - During the examination of the surveillance specimen brackets ( 003 and 177 degrees, both lower tack welds on the attachment bracket were observed to be cracked. This is located outside the Section XI examination boundary.

Note 7:
IRM/SRM - The dry tubes (IRM/SRM) were examined per GE SIL: 409. During this examination dry tube 38-35, SRM 'D', linear indications were noted above the tube to primary pressure boundary fillet weld, on the non-pressure boundary side. The indications have both horizontal and vertical components. This is not an ASME Section XI item. See attachment 2 for details.
Note 8:
I-MS-A-7PR-WA (Reference Number 100680) -- Portions of weld 1-MS-A-7PR-WA were not examined due to interference with the component support (main steam guide support). A relief request will be submitted to request relief from examining 100% of the examination volume. This relief request will include all four (4) main steam guide supports, one on each of the four (4) main steam piping lines.

Examinations:
Steam Dryer -
Clinton Power station has examined the steam dryer in accordance with General Electric SIL Numbers 474 and 644, Supplement 1. During CIROl in 1989 a crack was identified in the drain channel number 8 to the skirt. The length of the crack was 7 5/8 inches. Clinton Power Station evaluated this crack for continued operation and has since been monitoring for additional crack growth. During ClR08 in 2002 the crack length was same 7 5/8 inches. The steam dryer is not addressed in ASME Section XI.
Condition Noted - During CIR09 inspection of this crack revealed that it had grown 1 1/8 inches in one cycle.
Corrective Measures - Clinton Power station repaired this crack in CIR09 by performing under water welding.
Also, during CIR09 it was observed that the lower left stiffener has mechanical deformation.
Clinton Power Station performed an engineering evaluation and concluded that the stiffener still performs its intended function. There are no visible cracks in the base material or the attachment welds.
Steam Separator -
Condition Noted - During CIR09 examination of the separator's lower guide at zero degrees, it was observed that the left outside corners have mechanical deformation.
Corrective Measures - Clinton Power Station has evaluated this deformation and justified continued operation. There are no apparent cracks or missing pieces.
Core Spray Internal Piping -
In accordance with the VIP-18, BWR Core Spray Internals Inspection and Evaluation Guidelines, Ultrasonic examinations were performed on the accessible areas of the selected welds on Core Spray Internal Piping.
Condition Noted -- Indications were identified on three (3) of these welds, BP2, CP2, and DP2.
Two (2) of these welds, BP2 and CP2, have indications previously identified in CIR07 (2000) and continued operation was justified.
Corrective Measures - The VIP-18 document requires Clinton to take the following steps:
Expansion of sample - CPS has scheduled inspection of all accessible welds on the Core Spray Internal Piping. Therefore, no sample expansion is required.
Crack Growth Analysis - Crack Growth analysis was performed to demonstrate the structural integrity of the internal piping for continued operation for two (2) cycles.
Leakage Rate Calculation - Separate leakage rate calculations were performed to demonstrate that the required amount of water can be transported to the core with the existing condition including crack growth.
Steam Dryer Support Brackets -
Condition noted - During Cl R09 outage examination of the steam dryer support brackets, some of the brackets appear to have contact marks and some do not have any contact marks with the steam dryer.
Corrective Measures -- Clinton Power Station has evaluated this and justified continued operation.
There are no apparent cracks or missing pieces.
RPV Surveillance Specimen Brackets -
Condition Noted - During the ClR09 outage examination of the Surveillance Specimen Brackets located at 003 deg. And 177 deg., both lower tack welds on the attachment brackets were observed cracked.
Corrective Measures - Clinton Power Station has evaluated this and justified continued operation.
These tack welds are outside the examination boundary.
SRM'D'-
Condition Noted - During the CIR09 outage examination of SRM 'D' per GE SIL 409, linear indications were noted above the tube to Primary Pressure Boundary fillet weld, on the non-pressure boundary side. The indications have both horizontal and vertical components.
Corrective Measures - Clinton Power Station has evaluated this and justified continued operation for at least one cycle.
Snubbers Functional testing -
An initial sample (10% of each type) of fifty-nine (59) snubbers was selected from the population of 579 snubbers. During the performance of initial sample testing, three (3) snubbers (two type 1 and one type 2) failed to meet the acceptance criteria. Type 1 snubbers consist of PSA-1/4 and PSA-1/2 and type 2 snubbers consist of PSA-1, PSA-3, and PSA-10.
. An additional ten (10) type 1 snubbers were selected for functional testing. More snubbers failed to meet the acceptance criteria and additional snubbers, 5% for each failure, were selected for testing. In total thirty-nine (39) additional type 1 snubbers were selected for testing.
. An additional sixteen (16) type 2 snubbers were selected for testing. No failures occurred in this additional sample.
Overall, there were one hundred and sixteen (116) snubbers tested during ClRO9.
Type 1 Snubbers -
A representative sample of ten (10) type 1 snubbers was selected for functional testing during CIR09. Two (2) snubbers failed to meet the acceptance criteria (IMS38093S and IRB21596S).
Two condition reports (Issue Reports 201408 and 200369) were initiated to address these failures.
These two snubbers were disassembled for evaluation. The following identified the results of the evaluation:
. lMS38093S - The evaluation identified that grease in the snubber had hardened and the rods of the guide rod and bearing assembly were bent.
. IRB21596S - The evaluation identified that the shaft of the torque carrier and shaft assembly was bent.
Ten (10) snubbers (5% for each failure or 5 for each failure) were selected for testing. Three (3) snubbers (IMS10705S, IMS10909S, and IMS38028S) failed the acceptance criteria. A condition report (Issue Report 201819) was initiated to address the issue. These failed snubbers were disassembled and evaluated as follows:
All three snubbers - The evaluation identified evidence of water and heavy corrosion throughout the snubbers. This condition is similar to the condition caused by operation in a harsh environment (high temperature and humidity). These snubbers are located in the Aux. Steam Tunnel.
Based on this evaluation, CPS concluded that all type 1 snubbers located in the Aux. Steam Tunnel were potentially degraded and the remaining type 1 snubbers located in the Aux. Steam tunnel should be functionally tested. The following snubbers were the remaining type 1 snubbers located in the Aux. Steam Tunnel at the time of testing:

- IMS48003S These nine (9) snubbers were tested for extent of condition/condition monitoring purpose. This testing was in addition to the sample expansion criteria.
All of the above snubbers, except one (1), passed the testing criteria. One (1) snubber, IMS 1091OS, was found locked up and was not tested. A condition report (Issue Report # 202192) was initiated to address this issue. This snubber, IMS 10910S, was disassembled for evaluation. The results are as follows:
.1 MS 1091 OS - The evaluation identified that the bearing socket on the housing end of the carrier and shaft assembly was worn and the guide rods of the rod and bearing assembly exhibit wear marks.
No additional snubbers were selected due to the fact that the above snubbers were selected in addition to the expanded samples and for trending/life history purpose.
In addition to the nine (9) snubbers selected for trending purposes, fifteen (15) more type 1 snubbers (5 for each failure) were selected for testing. These snubbers were selected from other locations (other than Aux. Steam Tunnel). One snubber (IRB24575S) out of these fifteen (15) failed to meet the acceptance criteria. A condition report (Issue Report # 201885) was initiated to address the issue. This snubber, IRB24575S, was disassembled for evaluation. The following identified the results of the evaluation:
1RB24575S - The evaluation identified that the guide rods of the rod and bearing assembly were bent.
Five (5) more type 1 snubbers were selected for testing. These five (5) snubbers selected for testing are not located in the Aux. Steam tunnel. All five (5) passed the testing criteria.
Two (2) snubbers, IMS 10808S and IRI18002S, were identified in C1R08 as degraded and included in the CIR09 testing program for trending purposes. One (1) (lMS1080lS) of these two (2) snubbers failed to meet the acceptance criteria. A condition report (Issue Report 199563) was initiated to address this issue. This snubber was disassembled for evaluation. The results were:
- IMS 10808S - The evaluation identified that the bearing socket of the torque carrier and shaft assembly on the housing end was damaged. There was grease on the capstan and capstan spring, while the amount of grease did not appear to be excessive, it may have been sufficient to cause the high activation rates. This condition is similar to conditions caused by harsh environment.
This snubber is also located in the Aux. Steam Tunnel.
In summary, fifty-one (51) type I snubbers were removed from various systems for testing purposes; this included all type I snubbers from the Aux. Steam Tunnel building. There were a total eight (8) snubbers that failed the acceptance criteria.
As required by the ORM, piping components on which these snubbers are installed were analyzed to ensure the integrity of the piping component. The results were satisfactory and documented in Engineering Change (EC) 347493.
Type 2 Snubbers -
A representative sample of thirty two (32) type 2 snubbers were selected for functional testing during CIR09. One (1) snubber, IFC03081S, failed to meet the functional testing criteria. A Condition Report (Issue Report 199563) was initiated to address this issue.
The snubber was disassembled for evaluation. This evaluation identified the cause of the snubber being frozen to be an excessive amount of grease in the ball nut assembly. This grease had hardened to the point that it restricted the movement of the recirculating ball bearings thus causing the frozen condition. Dried grease was also found throughout the snubber.
Sixteen (16) additional snubbers (5% for each failure) were selected for functional testing. All snubbers passed the functional testing criteria.
As required by the ORM, the piping components on which this snubber was installed analyzed to ensure the integrity of the piping components. The following describes the results:
The snubber failed in both tension and compression directions during a drag test but passed its functional test. The snubber was degraded due to dried grease. The grease had hardened to the point that it restricted the movement of the re-circulating ball bearing thus causing frozen condition.
Based on the support drawing, the snubber will experience a thermal movement of 1/16" in tension direction. The frozen snubber would be locked in the tension direction thus restraining the pipe thermal movement; this would result in a higher piping stress which may exceed the code stress limits. However, due to the fact the potential thermal movement is 1/16", which is negligible in terms of piping analysis, the pipe and pipe supports would have insignificant impact due to the snubber locked out. No further evaluation is required. This was evaluated in the Engineering Change (EC) 347493.
As a result of implementing 4 modifications (MSF012, 341501, 341503, and 341504) eight (8) snubbers (IMS46001S. IMS48003S, IMS48005S, IMS10602S, IRT28006S, IRT28007S, 1RT31004S , and IRT46001S ) were removed permanently. The snubber population after CIR09 is 579-8 = 571.
It should be noted that CPS also tested a 10% sampling of type 3, 4, and 6 snubbers with no failures.

SUMMARY
NIS-2 OWNER'S DATA REPORTS This section provides a listing of the 70 ASME Section XI repairs or replacements performed between May 8, 2002 and February 25,2004.
All repairs and replacements are of a routine nature. CPS did not identify any flaws during performance of any ISI examination which required a repair or replacement.
CPS utilized Code Case N-416-1 during this reporting period. This Code Case allows the elimination of the hydrostatic testing requirement following welded replacement, providing some additional NDE is performed and the 1992 Edition of Section III used for examination.
CPS installed a number of modifications including MSFO12 on MSIVs, M079 on ECCS testable check valves, and the changeout of numerous Dresser Relief Valves with Crosby replacements.
Following the listing of repairs and replacements are copies of NIS-2 Owner's Data Reports for all of the Class 1, 2, and MC repairs and replacements which CPS performed during this reporting period.
