On 10/30/19 at 0026 EDT, an Unusual Event (HU3) was declared due to a release of flammable gas to the Unit 1 Pipe Penetration area (Reactor Auxiliary Building). The hydrogen supply valve to the area has been isolated. The area is being ventilated and follow-up air sampling will be performed. The NRC Resident Inspector has been notified. Notification of Unusual Event - Hurricane Warning

At time 1700 (EDT) today, the National Weather Service issued a Hurricane Warning for the area including the St. Lucie Nuclear Power Plant. On that basis, a Notice of Unusual Event was declared at time 1714 (EDT). In anticipation of severe weather, the site has completed proceduralized high wind preparations. Both units are currently stable at 100% power. The licensee notified both State and local agencies and has informed the NRC Resident Inspector. No safety-related systems are out of service. At 1701 (EDT) today (09/09/17), a 24-hour update was made to state and local governments following the National Weather Service issuing yesterday's hurricane warning for the area including St. Lucie Plant. On that basis, a Notice of Unusual Event was declared yesterday (09/08/17) at 1714 (EDT). There are no changes in plant status for either Unit. In anticipation of severe weather, the site previously completed high wind preparations. Our plants have been specifically built to withstand natural disasters such as hurricanes, tornadoes, earthquakes, flooding and tidal surges. This approach will ensure the safety of operations and our employees and the communities surrounding our plants. Both units are currently stable at 100% power level and will remain at that power since hurricane force winds are not currently projected to reach St. Lucie Plant. The NRC Resident Inspector has been informed. Notified R2DO (Michel).

Our plants have been specifically built to withstand natural disasters such as hurricanes, tornadoes, earthquakes, flooding and tidal surges. Unusual Event - Loss of Offsite Power

At 35 percent power, a main generator lockout caused the main generator to trip, resulting in a reactor trip of Unit 1. Because of the lockout, power did not transfer to the startup transformers. Both emergency diesel generators started and aligned to the emergency busses. During the trip all control rods fully inserted and no safety or relief valves lifted. The plant is in Mode 3 steaming through the atmospheric relief valves and feeding the steam generators using auxiliary feedwater. There is no reported primary to secondary leakage. Primary coolant is being moved using natural circulation cooling. The trip of Unit 1 had no effect on Unit 2. The licensee notified the NRC Resident Inspector. Notified DHS SWO, FEMA, DHS NICC, and Nuclear SSA (via e-mail).

The Unusual Event was terminated at 2125 EDT after the plant restored normal offsite power. The licensee notified the NRC Resident Inspector. Notified the R2DO (Sandal), IRD (Gott), NRR EO (Miller), DHS SWO, FEMA, DHS NICC, and Nuclear SSA (via e-mail).

On August 21, 2016 at 1926 EDT, St. Lucie Unit 1 experienced a reactor trip and a loss of offsite power due to a main generator inadvertent Energization Lockout Relay actuation. The cause of the lockout is currently under investigation. Coincident with the loss of offsite power, the four reactor coolant pumps deenergized. Both the 1A and 1B Emergency Diesel Generators started on demand and powered the safety related AC buses. All CEAs (Control Element Assemblies) fully inserted into the core. Offsite power to the switchyard remained available during the event, and at 2036, restoration of offsite power to St. Lucie Unit 1 was completed. Decay heat removal is being accomplished through natural circulation with stable conditions from Auxiliary Feedwater and Atmospheric Dump Valves. Currently maintaining pressurizer pressure at 1850 psia and Reactor Coolant System temperature at 532 degrees F. St. Lucie Unit 2 was unaffected and remains in Mode 1 at 100% power. This report is submitted in accordance with 10 CFR 50.72(b)(2)(iv)(B) for the reactor trip and 10 CFR 50.72(b)(3)(iv)(A) for the Specified System Actuation. The licensee notified the NRC Resident Inspector. Notified the R2DO (Sandal).

On August 21, 2016 at 2330 EDT, St. Lucie Unit 1 started two Reactor Coolant Pumps to establish Forced Circulation in order to enhance Decay Heat removal. Plant conditions remain stable with Auxiliary Feedwater and Atmospheric Dump Valves in service. This report is submitted in accordance with 10 CFR 50.72(c)(2)(ii) as a follow up notification of protective measures taken. The licensee notified the NRC Resident Inspector. ENS 4971210 January 2014 17:14:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Temporary Bypass of Normal Outfall During Severe WeatherOn January 10, 2014, Florida Power and Light reported to the Florida Department of Environmental Protection that the St. Lucie Nuclear Plant temporarily bypassed the normal storm collection outfall path at 17:45 EST on January 9, 2014. The bypassed flow of water was a result of severe rainfall at the station and a blockage in the normal outfall path. This non-emergency notification is being made pursuant to 10CFR50.72(b)(2)(xi) due to the notification of the Florida Department of Environmental Protection. The licensee originally made the offsite notification to the State of Florida agency associated with the emergency declaration and severe weather event on January 9, 2014 (see Event Notification 49707). The licensee has notified the NRC Resident Inspector.

On January 9, 2014 St. Lucie Unit 1 declared an Unusual Event based on a severe rain storm, a natural occurrence affecting the Protected Area. Specifically, visual sightings by station personnel that water levels are approaching storm drain system capacity. Countermeasures are being deployed and are currently operating satisfactorily. Both units continue to operate at full power. All safety systems remain available. The licensee has notified the NRC Resident Inspector, state, and other local authorities. ENS 491788 July 2013 12:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Sodium Hypochlorite Leak:On July 8, 2013, at 0830 EDT, the National Response Center and the State Watch office were notified of a leak greater than the limit of 109 gal of 11% Sodium Hypochlorite onto the ground. Total amount spilled is approximately 200 gallons. While the leak is currently active, the leak has being routed to an approved injection point. This notification is being made in accordance with 10CFR 50.72 (b)(2)(xi) due to notification of off-site agencies. The NRC Resident Inspector has been notified.
ENS 4758310 January 2012 16:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to State of Florida Due to Exceeding Wastewater Discharge Permit LimitFlorida Department of Environmental Protection was notified at 1130 EST on 01/10/12 that Stormwater Basin Discharge for the month of December exceeded the total monthly allowable limit for Total Suspended Solids (TSS). The Monthly Average for TSS limit is 34 mg/l. The limit per the St. Lucie Plant DEP Discharge Permit is 30 mg/l for the month. This limit was exceeded due to rainfall and required pumping prior to an additional sample point. While the basin was pumped, the Daily Maximum Limit of 100 ppm was not exceeded. The FDEP representative concurred that there was no threat to the public as a result of this discharge. This non-emergency notification is being made pursuant to 10 CFR 50.72(b)(2)(xi) due to the Offsite Notification to the Florida Department of Environmental Protection. The licensee informed the NRC Resident Inspector.

Manual Reactor Trip Due to Rising Condenser Backpressure

On August 22, 2011 at 1513 (hrs. EDT), Unit 1 was manually tripped due to rising condenser backpressure. All CEAs fully inserted into the core. Decay heat removal was initially from main feedwater and steam bypass to the main condenser. The cause of the rising back pressure was an influx of jellyfish into the intake structure, degrading the circulating water system performance. Subsequent to the manual trip, the 1B Main Feedwater Pump was manually secured due to a leak on the pump casing. The 1A Main Feedwater Pump subsequently tripped due to low suction pressure after manually securing the 1B Condensate Pump, per procedure. Decay heat removal was transitioned to atmospheric dump valves and auxiliary feedwater. Unit 2 is in Mode 1, currently at 70 % power. Unit 2 power is being reduced from 100% in response to the influx of jellyfish. This event is reportable pursuant to 10 CFR 50.72(b)(2)(iv)(B) for the reactor trip. During the transient, no primary or secondary relief valves lifted. Offsite power is stable and the plant is in its normal shutdown electrical line-up with power being supplied from offsite. There is no known primary-to-secondary leakage. The cause of the 1A Main Feedwater Pump trip is under investigation. Unit 2 remained at 70% reactor power before and after the event. The licensee has notified the NRC Resident Inspector.

On August 22, 2011 an abnormal fish kill of at least 1000 lbs was observed in the combined unit's intake canal. The cause of the fish kill was related to an unusually large sustained influx of jellyfish into the intake canal. Per the plant's environmental permit, the Florida Fish and Wildlife Conservation Commission (FWCC) was notified at 1627 EDT. This non-emergency notification is being made pursuant to 10 CFR 50.72(b)(2)(xi) due to the notification of the FWCC. The licensee has notified the NRC Resident Inspector.

ENS 454157 October 2009 07:20:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to the State of Florida and the National Response CenterThis notification is being made pursuant to 10 CFR 50.72(b)(2)(xi) due to notification of other government agencies at 0320 EDT on 10/7/09. Florida Power and Light Company (FPL) has identified a Sodium Hypochlorite spill within the protected area at Plant Saint Lucie. The spill has been terminated and it is estimated that up to 240 lbs of Sodium Hypochlorite was spilled past the secondary containment of the hypochlorite tank. The precise volume of the spill is indeterminate at this time. This event poses no health or safety consequences to either on site personnel or the general public. Notification to the State of Florida Emergency Response Commission and the National Response Center was made in accordance with the Plant Saint Lucie hazardous material spill response procedure. ENS 448252 February 2009 20:45:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to the Environmental Protection AgencyOn 02/02/2009, at 1545, the Environmental Protection Agency (EPA), Atlanta, Georgia, was notified of a loss of approximately 85 pounds of refrigerant R-22 (Chlorodifluoromethane) from a crack in a sensing line on the Unit 1 Control Room Air Conditioner, ACC-3B. The 85 pounds of refrigerant represents the entire charge in the air conditioner and exceeds the limit specified in 40 CFR 82.156. The EPA was notified as required by the station's Environmental Reporting procedures. This non-emergency notification is being made pursuant to 10 CFR 50.72(b)(2)(xi) due to the notification made to the EPA. The licensee has notified the NRC Resident Inspector.
ENS 4443722 August 2008 20:20:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Hydrazine SpillAt 1620 EDT, the following off-site agencies were notified of approximately 1.8 gallons of 35 weight-percent Hydrazine spillage into the storm drain system: National Response Center, State Emergency Response Commission, and Florida Department of Environmental Protection. The storm drain system discharges to an on-site settling pond. The amount of Hydrazine spilled was greater than the reportable quantity of 0.34 gallons of 35 weight-percent hydrazine. The leak has been identified and stopped, area has been cleaned. Notification due to 10CFR50.72(b)(2)(xi), due to notification of off-site agencies. The licensee notified the NRC Resident Inspector.
ENS 4392924 January 2008 03:15:0010 CFR 50.72(a)(1)(i), Emergency Class DeclarationUnusual Event - Uncontrolled Hydrogen LeakSt. Lucie Plant made an Unusual Event notification to the Florida State Warning Point @ 2228 EST, 01/23/08 based on a Hydrogen Tube Trailer fitting failure, and subsequent leak of hydrogen gas. The emergency classification was Emergency Action Level #20. A., per St. Lucie Plant EPIP-01, 'Classification of Emergencies'. The hydrogen leak was terminated upon discovery, with no subsequent fire and no personnel injuries. The event (and declaration) was terminated @ 2228 EST, on 01/23/08. The licensee notified the NRC Resident Inspector, and the Commonwealth of Florida Warning Point.
ENS 4372315 October 2007 13:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to Florida Department If Environmental Protection (Fedp)At 0950, the Florida Department of Environmental Protection (FDEP) was notified in regards to a Turbine Lube Oil spill of approximately 350 gallons. The notification, to the FDEP, was required due to the inability to reach clean-up target levels within 30 days. In addition, the State Warning Point was notified at 1010. This non-emergency notification is being made, pursuant to 10 CFR 50.72(b)(2)(xi), due to the notification of outside government agencies in response to this environmental event. ENS 4352927 July 2007 16:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Hydrazine SpillAt 1200 EDT, the following off-site agencies were notified of 0.68 gallons of 35 weight-percent Hydrazine spillage into a concrete berm: National Response Center, State Emergency Response Commission, and Florida Department of Environmental Protection. The amount of Hydrazine spilled was greater than the reportable quantity of 0.34 gallons of 35 weight-percent hydrazine. As stated above, the leak was fully contained in its designated containment area and none reached the environment. The leak has been identified and stopped, area has been cleaned. ENS 4280425 August 2006 19:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseTritium Found in an Onsite Settling Pond

This notification is being made pursuant to 10 CFR 50.72(b)(2)(xi) due to notification of other government agencies at 15:30 on 8-25-06. Florida Power & Light Company (PPL) has identified a small amount of tritium in a settling pond at its St. Lucie Nuclear Plant Site. The level of tritium detected in the settling pond, 3.0 E-6 microCuries per milliliter (uCi/ml), is well below the Environmental Protection Agency's (EPA) drinking water standards of 2.0 E-5 uCi/ml and is less than two per cent of the Nuclear Regulatory Commission's (NRC) safe standards for tritium that apply to water in an unrestricted area. The source of the tritium is under investigation. The most likely source was from a leaking drain valve off a recirculating line associated with the Refueling Water Storage Tank. The leak has been stopped. This discovery presents no health or safety risk to employees or the public.

The St. Lucie Plant does not have drinking water wells on site and the plant

does not impact drinking water off site. The settling pond is a part of the normal drainage process at St. Lucie. Any water that is released from the settling pond is sampled for radioactivity and the results of any findings are included in a Radioactive Effluent Release Report that is filed annually with the NRC. Monitoring of the settling pond is conducted on a monthly basis. The previous measurements of the pond water on July 20, 2006 indicated that there was no tritium present. Notification to the State of Florida and St. Lucie and Martin county agencies has been reported in accordance with the Nuclear Energy Institute's voluntary reporting initiative. The licensee notified the NRC Resident Inspector.

ENS 427522 August 2006 20:00:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseHazardous Material Disposed from Owner Controlled Area(On) 08/02/06 at 1600 hrs. EST, Plant Security officers discovered a canister marked: 'Flammable Contains Phosphorus Notify Military or Law Enforcement if Found,' on plant owner-controlled property, outside the protected area. St. Lucie County Sheriffs Office (SLCSO) was contacted for determination of hazard posed. According to Plant Security supervision, the canister did not, and does not pose a threat to plant personnel, the public, or plant operations. SLCSO has asked other local experts to examine the canister and arrange for satisfactory disposal. The licensee notified the NRC Resident Inspector.
ENS 423906 March 2006 17:20:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Lube Oil SpillOn 03/06/06 at 10:40 (EST) a plant operator was dispatched to investigate an unexpected Annunciator D-43, 'TURBINE LUBE OIL RESERVIOR LEVEL HIGH/LOW'. He reported the alarm was a valid low reservoir level (for the main turbine). A contractor painter had inadvertently bumped open a drain valve on the Kaydon turbine oil filter system. The valve has been closed. The spilled oil was contained within the Unit 1 Turbine Lube Oil Secondary Containment Structure. The volume of spilled oil has been calculated to be approximately 200 gallons. The Florida Department of Environmental Protection. (DEP) was notified on 3/6/06 at 12:20 EST in accordance with plant procedures for oil spills greater than 25 gallons on a pervious surface. No equipment damage was incurred due to the loss of reservoir level. Actions to retrieve the spilled oil are in progress. ENS 421051 November 2005 03:23:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseContainment Evacuation and Smoke Inhalation Treatment Due to Small FireOn October 31, 2005, at 22:23 fire and smoke was reported in the Unit 1 Reactor Containment Building. Unit 1 is currently shut down for a refueling outage. Investigation into the cause of the fire continues. The fire appears to have been caused by slag from cutting/grinding work contacting a temporary ventilation hose. The containment evacuation alarm and fire alarm was sounded. The fire was reported extinguished at 22:25. The fire team and first aid team responded. First aid team reported several people were overcome by smoke inhalation. The Unit 1 control room called '911' to request ambulance and EMT assistance. Eight people have been transported to local hospitals for treatment of smoke inhalation. None of the people transported offsite were contaminated. 29 CFR 1904.8 requires notification of the Occupational Safety and Health Administration (OSHA) within eight hours of this event because it resulted in the hospitalization of three or more employees as a result of a work related incident. This NRC notification is being made in accordance with 10 CFR 50.72(b)(2)(xi) due to the notification of offsite governmental agencies via '911' as well as the OSHA notification. Unit 2 is at 100% power and was not impacted by this event. The licensee notified the NRC Resident Inspector.
ENS 4209831 October 2005 00:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite NotificationOn October 30, 2005, at 1950 an altercation occurred between two contract personnel. The event occurred inside the protected area, but outside the radiation controlled area. The event took place in a temporary building used as a dress out area for the Unit 1 refueling outage. One of the two people suffered a minor cut to his arm. There was no contamination related to this incident. FPL Security escorted the two individuals off site and revoked their plant access. The local St. Lucie County Sheriff department was called and is investigating the event. One person has been arrested for simple battery and is in custody of the St. Lucie County Sheriff Department. A Hurricane Warning has been issued for the southeast Florida coast from Vero Beach southward to Florida City. A Hurricane Warning means that Hurricane conditions are expected with the warning area within the next 24 hours. The St. Lucie Plant falls within this Hurricane Warning area. The licensee entered the Unusual Event based on EAL '5-B.' Currently Unit 2 'B' Train of Component Cooling Water is out if service for planned maintenance. The licensee notified the state and the NRC Resident Inspector.

ENS 4130131 December 2004 04:55:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseHydrazine Release to Settling Pond Exceeded Allowed Release RateAt 2355 on 12/30/04, the National Response Center was notified that the environmental release limit for hydrazine had been exceeded. A maximum of 3.8 pounds of hydrazine was released over an 8 hour period which exceeded the reportable quantity of one pound in 24 hours. A feed and bleed of the condensate system was in progress in preparation for plant startup. The release limit was exceeded due to a higher than expected concentration of hydrazine in the condensate system. The cause of the event is under investigation. The hydrazine addition was secured and draining of the system has been terminated. All of the hydrazine has been contained in a settling pond within the protected area. ENS 4123129 November 2004 18:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to 50 Gallon Hydraulic Oil Spill OnsiteOn 11/29/04, a leased mobile crane discharged approximately 50 gallons of hydraulic oil through a faulted line as maintenance contractors were performing pre-operational checks. The oil spill was confined to the compacted shell rock ground area beneath the crane. Immediate actions were taken to clean and remove the spilled oil using absorbent material and by removing saturated soil. The magnitude of the spill required off-site notification to the Florida Department of Environmental Protection (FDEP) and to the St. Lucie County Health Dept. The licensee will inform the NRC Resident Inspector.
ENS 4112114 October 2004 20:10:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification for Exceeding State Permit LimitsAt 1610 EDT on 10/14/04, Florida Power and Light Company notified the Florida Department of Environmental Protection (FDEP) that St. Lucie Station exceeded a permit allowance limitation for operating time on one of the station's administrative office building emergency diesel generators. FDEP notification was required per the general conditions of the permit. The permit allows 200 hours of run time per year. This particular generator logged 222.9 hours of run time in September 2004. The additional usage was directly related to Hurricane Frances and Jeanne. This particular generator was used to supply the South Service Building with electricity during the two hurricanes. Unusual Event Declared Due to a Loss of Offsite Power

At 2356 (EDT), September 25, 2004, off-site power was lost for both units. At the time of the event both units were in mode 4 and cooling down to meet shutdown entry conditions. The shutdown of both units was due to hurricane conditions from hurricane Jeanne. All four emergency diesels (2 per unit) started and properly loaded. Both units are stable in natural circulation cooling. Efforts continue to place both units on shutdown cooling. All systems operated as expected. The plant was already in a Notification of Unusual Event due to the Hurricane. Shutdown Cooling was established on Unit 1 at approximately 0020 EDT on 9/26/04. The licensee informed both state and local agencies and the NRC Resident Inspector. A conference call was held at approximately 0005 EDT with R2 Response Manager (Len Wert), NRR EO (Stu Richards) and IRD (Susan Frant) to discuss the loss of offsite power. The participants concluded that the NRC Monitoring Mode entered at 1515 EDT on 9/25 for the Hurricane was appropriate.

At the time of the LOOP all four emergency diesel generators started and loaded the safety related buses. In it's initial notification of the LOOP (0035 09/26/04, EN# 41067), FPL reported that all systems had performed as expected. However, during it's post LOOP walkdowns of the control room boards the control room operators determined that the 1 B Intake Cooling Water (ICW) pump had not automatically started as expected. The pump was subsequently started from the control room using the manual control switch. The Reactor Coolant System (RCS) leakage was greater than ten gallons per minute as indicated in the Control Room. The Facility declared an Unusual Event at 1856 EST since the RCS leakage was greater than ten but less fifty gallons per minute. Refueling activities were stopped as a result of the incident. The RCS leakage was isolated and the Unusual Event was terminated at 1915. The licensee has notified local, state and other government agencies through the ring down phone system. The turtle was found on the surface at the barrier net with no injuries or abnormalities except for fresh cuts common for turtles coming through pipes. The cause of death is unknown at this time. At approximately 12:45 p.m. on 6/17/03 a loggerhead turtle was rescued from the St. Lucie Plant intake canal. The turtle was injured and will be transported to the State authorized sea turtle rehabilitation facility. The Florida Dept. of Environmental Protection (FDEP) was notified at 1:09 p.m. as required by the St. Lucie Plant Sea Turtle Permit - see the attached sea turtle stranding report (1 page). Notification to the state government agency requires a four (4) Hr. Non-Emergency Notification to the NRC per 10 CFR 50.72(b)(2)(xi). There were no unusual plant evolutions in progress which may have contributed to this event. The licensee notified the NRC Resident Inspector. Notified R2DO (L. Wert).

The licensee was informed by the FWCC that the green sea turtle requiring rehabilitation which was retrieved from the plant's intake canal on 3/15/03 had expired within 10 days of removal. Consequently, the licensee is updating their 3/15/03 report to include this additional information. The licensee will inform the NRC Resident Inspector. Notified R2DO(C. Ogle).

On 04/02/03, a green sea turtle was retrieved from the intake canal. The turtle required rehabilitation. The injuries were not causal to plant operation. Per the plant's turtle permit, the Florida Fish and Wildlife Conservation Commission was notified. This non-emergency notification is being made pursuant to 10 CFR 50.72 (b)(2)(xi) due to offsite notification. On 07/01/03 licensee QA discovered that the injury had occurred and that the NRC had not been notified within the 4 hours. The failure to report the event within the regulatory time is being investigated and has been entered into the stations corrective action program. The licensee notified the NRC Resident Inspector. The injuries to the turtle are not causal to plant operation. Per the plant's Turtle permit, the Florida Fish and Wildlife Conversation Commission (FWCC) was notified at 1715 EDT. This non-emergency is being made pursuant to 10 CFR 50.72(b)(2)(xi) due to the notification of FWCC. The licensee notified the NRC Resident Inspector. Notified R2DO (Munday).

On 10/7/03, at 08:30, a Dead Green sea turtle was retrieved from the plant's intake canal. There were no abnormalities or injuries found. The death does not appear to be causal to plant operation. The turtle will be sent to veterinarian for necropsy. Per the plant's Turtle permit, the Florida Fish and Wildlife Conservation Commission (FWCC) was notified at 0849 EDT. This non-emergency notification is being made pursuant to 10CFR 50.72(b)(2)(xi) due to the notification of FWCC. Notified R2DO (K. Landis) and the licensee notified the NRC Resident Inspector.
