The leak was able to be isolated and the area is being cleaned up. This event is reportable within 4 hours per 10 CFR 50.72(b )(2)(xi) any event or situation related to protection of the environment for which a notification to other government agencies has been made. ENS 4985022 February 2014 21:37:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Concerning a Small Hydraulic Fluid Spill

(The New Jersey Department of Environmental Protection) and Ocean County Health (Department were) notified of a spill of approximately 1 cup of hydraulic fluid (oil) that leaked from a vehicle and entered a storm drain at approximately 1615 (EST). The licensee notified the NRC Resident Inspector.

Upon subsequent review of the Material Safety Data Sheet associated with the fluid that leaked from the vehicle, it was determined that the fluid was not a hazardous substance as defined by the State of New Jersey (Appendix A of N.J.A.C. 7:1E). Additionally, it was determined that the fluid did not reach surface water. Based on this information, the New Jersey Department of Environmental Protection (NJDEP) was notified that the fluid that leaked from the vehicle did not reach surface water and was not a hazardous substance. The NJDEP notification was updated for the record. For the above reasons, the 4 hour non-emergency ENS notification made under EN # 49850 is hereby retracted The NRC Resident Inspector has been informed. Notified R1DO (DeFrancisco).

The spill has been contained and cleaned-up. There was no actual or potential impact on the environment and no impact on the health and safety of the public or onsite personnel. The spill did not reach the federal reportable quantities limit. Due to notification of a government agency, this event is being reported under 10CFR50.72 (b)(2)(xi). Unusual Event Due to Loss of Offsite Power

At 0329 EDT, Oyster Creek experienced a loss of offsite power. As a result of the loss of offsite power, the unit automatically scrammed from 100% with all control rods fully inserting and all safety systems functioning as required. Both Emergency Diesel Generators automatically started and are carrying loads on the safety buses. At 0341 EDT, Oyster Creek declared an Unusual Event based on a loss of offsite power for greater than 15 minutes. The unit is stable in Hot Shutdown with decay heat removal via the Isolation Condenser. The cause of the loss of offsite power is currently under investigation by JCP&L. The licensee notified the State and local agencies, as well as the NRC Resident Inspector. The licensee will be making a press release.

Oyster Creek has terminated from the loss of offsite power Unusual Event (at 0538 EDT). All safety systems functioned as expected for this event. The licensee has notified the NRC Resident Inspector. Notified R1DO (Dimitriadis), NRR EO (Nieh), IRD (Gott), DHS SWO, FEMA, and DHS NICC.

As a result of the loss of offsite power, both Emergency Diesel Generators automatically started and carried loads on the safety buses, as required. Offsite power was returned to service at 0457 EDT and both Emergency Diesel Generators were secured at 0520 EDT. Per 50.72(b)(3)(iv)(A), Oyster Creek is reporting any event or condition that results in valid actuation of an Emergency AC electrical power system. Additionally, the Reactor Building (Secondary Containment) differential pressure indicated positive 0.25 inches W.G. at approximately 0357 EDT. Reactor Building differential pressure indication returned to normal at 0434 EDT. Oyster Creek is currently investigating the cause of the positive Reactor Building pressure indication. Per 50.72(b)(3)(v)(C), Oyster Creek is reporting an event that could have prevented the fulfillment of the safety function of a system needed to control the release of radioactive material. The licensee notified the NRC Resident Inspector. Notified R1DO (Gray).

As a result of further investigation it was found that the Reactor Building Differential Pressure issue was an indication issue only and not indicative of a loss of the secondary containment barrier. The positive indication was caused by a degradation of instrument air pressure to the instrument used to generate the differential pressure indication. The degradation of instrument air pressure was an expected condition caused by the loss of offsite power. Alternate indication, not affected by instrument air pressure, was available throughout the event. The alternate indication read negative 0.4 Inches water gauge throughout the event. Based on this information Oyster Creek is retracting the portion of this report that was reported per 50.72(b)(3)(v)(C), an event that could have prevented the fulfillment of the safety function of a system needed to control the release of radioactive material. The licensee has notified the NRC Resident Inspector. R1DO (Newport) notified.

Intermittent Signal Interference with Emergency Sirens

At 12:00 on July 13, 2012 it was determined, during a conference call with the Exelon siren vendor, that Oyster Creek emergency planning zone (EPZ) sirens may be unreliable. The unreliability is being caused by an intermittent signal on the frequency used to activate the sirens. Therefore, an 8-hour report is required under 10 CFR 50.72(b)(3)(xiii) due to a potential 'Loss of Emergency Preparedness Capabilities.' Additionally, Exelon is contacting the Federal Communications Commission (FCC) for troubleshooting and reporting purposes, as well as, the New Jersey Office of Emergency Management (NJ OEM) and the Ocean County Office of Emergency Management to institute compensatory actions. Due to contacting other government agencies a 4-hour report is required per 10 CFR 50.72(b)(2)(xi). Exelon will notify the NRC Operations Center upon full-restoration of the Oyster Creek EPZ siren system. The NRC Resident Inspector has been informed.

As of 2058 EDT on July 14, the licensee has declared that all 42 sirens have been restored to a reliable status. The siren vendor has shifted the activation frequency to avoid the interference. The vendor is still working with the FCC to determine the cause of the interference signal at the original frequency. The licensee has notified the NRC Resident Inspector. R1DO (Doerflein) notified.

ENS 4805227 June 2012 20:30:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseInadvertent Actuation of an Emergency SirenAt 1630 EST, Exelon was notified by the NJ State OEM (Office of Emergency Management) that Oyster Creek Emergency Preparedness Siren #25, located in Harvey Cedars, NJ, was inadvertently actuated at 0209 EST and sounded for a period of 2-3 minutes. The siren was inadvertently actuated by Atlantic City Electric when restoring power to the siren. No actual plant emergency existed. Siren #25 and all other of the 42 sirens remain functional. No other reportability thresholds have been met or exceeded. State and local authorities were also notified. The licensee will notify the NRC Resident Inspector.

Technical Support Center Out of Service and Offsite Notification Due to Loss of Power to Site Emergency BuildingAt 0607 hrs. EDT, a vehicle struck a pole on Route 9, immediately in front of the site, causing a loss of power to the Technical Support Center (TSC). At 0615 hrs., the Fire Brigade was dispatched to investigate reports of a 'smoldering' transformer behind the Site Emergency Building (SEB) and Oyster Creek Administration Building (OCAB.) At approximately 0708 hrs., the on-scene Fire Brigade Leader called the local fire department, because, in his determination, it was an event or situation that was related to the health and safety of the onsite personnel. At 0717 hrs., the local fire department came on-site in response to the call. Upon investigation, it was found that the transformer was smoking due to overheating. There was no fire and no Emergency Action Level (EAL) was applicable for this event. Due to the notification and response of the local fire department, a 4-hour report is required per 10CFR50.72(b)(2)(xi). At 0731 hrs., power was restored to the TSC and the TSC was restored to a fully operable condition. Due to the loss of the power to the TSC for greater than one hour (time required for TSC staffing), an 8-hour report is required in accordance with 10CFR50.72(b)(3)(xiii). The NRC Senior Resident was on-site and notified of the event.
ENS 4752012 December 2011 21:50:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification to State of New Jersey Due to High Opacity of Diesel ExhaustAt 1447 (EST) Oyster Creek experienced a failure of a discharge flange on a fire protection pond pump. The fire system backup fire diesel started in response to low system pressure. At 1540 (EST), it was noted that the fire diesel had higher than expected opacity from its exhaust. High exhaust opacity (equal to or greater than 20% for more than 3 minutes in any consecutive 30 minute period) is reportable to the State of New Jersey in accordance with the Auxiliary Boiler Air Pollution Control Permit. At 1650 (EST), high opacity from the fire diesel was reported to the State of New Jersey via the NJ Department of Environmental Protection hotline. In accordance with 10 CFR 50.72 (b)(2)(xi) this event is being reported due to Offsite Notification. The licensee has notified the NRC Resident Inspector.

ENS 4710729 July 2011 01:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Fish Kill EventAt 2115 on 7/28/2011, Oyster Creek experienced a fish kill of greater than 100 fish caused by a loss of the thermal dilution plant power. The loss of thermal dilution plant power was based on a faulted breaker in the Oyster Creek/JCP&L substation. The reported failure was a relay failure inside a 34.5KV substation breaker. Oyster Creek is holding at 53% power until thermal dilution is restored for environmental limitations. The New Jersey Department of Environmental Protection has been notified. The NRC Resident Inspector will be notified.
ENS 4649520 December 2010 13:05:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseFish Kill Caused by Rate of Temperature Change in Discharge CanalAt 0938 hours (EST), the New Jersey Department of Environmental Protection (NJDEP) was notified of a fish kill. The fish kill is due to the rate of temperature change in the discharge canal, associated with an unscheduled and non-routine plant shutdown for a reactor recirculation pump repair. As of 0930 hours, seven (7) Speckled Sea Trout and two (2) Black Drums were reported dead west of the route 9 bridge in the stations discharge canal. The (New Jersey Pollutant Discharge Elimination System) NJPDES discharge to surface water permit No. 0005550 part IV-B/C item 11, rate of temperature change states, "The rate of temperature change shall not cause mortality of fish or shellfish. This report is being made in accordance with 10 CFR 50.72(b)(2)(xi) for the occurrence of any event or situation related to protection of the environment, for which a news release is planned or notification to other government agencies has been made. Oyster Creek Nuclear Generating Station notified the New Jersey Department of Environmental Protection of a leak of water containing tritium from an underground condensate transfer pipe at the turbine building penetration of less than 20 gpm and concentration of approximately 10 million picocuries/milliliter. The leaking water is being pumped to storage drums and a plan is being developed to replace the six-inch condensate transfer piping which penetrates the turbine building inside a 42 Inch long sixteen inch diameter penetration sleeve. Previously, water was observed coming from the sleeve into the turbine building where it was draining to a sump. The leak coming out of the turbine building was uncovered during excavation to determine if the condensate was leaking to the soil. The licensee notified the NRC Resident Inspector.

ENS 4499315 April 2009 20:44:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to Potential Release of TritiumOyster Creek notified the New Jersey Department of Environmental Protection of a potential release due to the discovery of detectable levels of Tritium in a cable vault that was found while replacing the cable to an (Emergency Service Water) ESW pump. No release is known to be occurring and no release is known to have occurred. The source of the Tritium water is not currently known. A prompt investigation is in progress. The licensee has notified the NRC Resident Inspector.

Automatic Reactor Scram Due to Main Transformer Fault

At 98% power, a reactor scram occurred due to a Main Transformer fault. The plant is currently stable in the Hot Shutdown Mode. Isolation Condensers initiated as part of this event are currently in Standby Mode. This event is reportable in accordance with 10CFR50.72(b)(2) and (b)(3) and the Station Reportability Manual section SAF 1.6 and SAF 1.7. The cause of the transformer fault is unknown and under investigation. There was no fire or explosion associated with the fault. All rods inserted during the scram. As a result of the transient, one electromatic relief valve lifted and subsequently reseated. There were no other isolations or safety equipment actuations during the transient. The station electrical system is in its normal shutdown lineup with offsite power available. Decay heat is being removed via the turbine bypass valves to the main condenser. Pressure is currently at 700 psi with water level being maintained at 160" via the main feed system. The licensee has notified the NRC Resident Inspector and will be notifying the State of New Jersey.

ENS 4461229 October 2008 23:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Due to a Spill of a Hazardous SubstanceThe State of New Jersey was notified of a spill to the ground of a hazardous substance. A tube leak in a new radwaste heat exchanger caused an expansion tank to overflow resulting in less than one (1) gallon of water containing corrosion inhibiting chemicals to be discharged to the ground. The heat exchanger and the new radwaste service water system were removed from service, stopping the leak. Initial samples did not detect radioactivity in the water. Cleanup of the spill is in progress. The licensee notified the NRC Resident Inspector.Service water

The cause of the spill was due to the deliveryman having trouble with the hose connection on the truck. The licensee notified the NRC Resident Inspector.

Manual Reactor Scram Due to Lowering Reactor LevelAt 1145 hrs. EST, operators inserted a manual reactor scram due to lowering reactor water level after the 'A' main feedwater pump tripped. Prior to the scram, the plant was operating at reduced power due to removing the 'B' main feedwater pump from service for planned maintenance. Reactor water level was at 155" when the scram was initiated and lowered to 100" during the transient. Currently, reactor water level is at the normal operating band and is being maintained by the feedwater system. After the scram, all safety systems functioned as expected. There were no system isolations due to the transient. Electrical power is being provided by normal offsite sources via the startup transformer. Decay heat is being removed via the normal path through bypasses to the main condensers. Additionally, the licensee made an offsite notification to the New Jersey Department of Environmental Protection due to approximately 300 dead or distressed fish in the discharge canal. Notified New Jersey D.E.P. (Department of Environmental Protection) of a spill of ethylene glycol in the Site Emergency Building at Oyster Creek. The spill was reported to NJDEP in accordance with N.J.A.E. 7:1.E, 7:26. The spill was contained and cleaned. The spill occurred during work on the Site Emergency Building air conditioning system. The licensee notified the NRC Resident Inspector.

At 11:55 on 7/19/07, New Jersey Dept. of Environmental Protection (NJDEP) was informed that: The spill of ethylene glycol, reported to them on 07/18/07, was subsequently determined to be a substance other than ethylene glycol; it was not a hazardous substance; and it was not discharged to the environment. Therefore, no report was required and the NJDEP notification (on 07/18/07) was retracted. For the above reasons, the 4 hour non emergency ENS notification , made under EN # 43502, is hereby retracted. NRC R1DO (Jim Krafty) notified. The NRC Resident Inspector was notified of this retraction by the licensee.

Automatic Reactor Scram on Low Reactor Vessel Water Level (Rvwl)

An automatic reactor scram occurred at 0522 on 07/17/07 due to a low reactor water level, following a trip of the 'C' Reactor Feed Pump. Reactor water level lowered to the low-low level setpoint and a reactor isolation occurred. Reactor level has been returned to the normal band and a reactor cooldown has been initiated using isolation condensers. The cause of the "C" RFP trip is attributed to an electrical fault as sensed on ground sensing and differential sensing instrumentation. No flames or smoke was observed, however, operators reported an acrid smell of insulation. Following the automatic scram on low reactor water level (138" above top of active fuel (TAF)), level continued to decrease to the low-low setpoint of 86" TAF resulting in a reactor isolation signal. The reactor isolation signal initiates closure of the Main Steam Isolation Valves (MSIVs), trip of the Recirc Pumps and actuation of the Isolation Condensers. Current reactor pressure is 520 psig with a cooldown rate of 82F/hr. The licensee plans to issue a press release and will take the Unit to cold shutdown for repairs. The licensee informed the NRC Resident Inspector.

The following information reported under 10CFR50.72(b)(3)(iv)(A) was provided as an update: The following systems actuated upon receipt of a lo-lo reactor water level signal, subsequent to the reactor scram, reported under EN #43495, which occurred at 0522 on 07/17/07: MSIVs - Isolation Isolation Condensers - Initiation Standby Gas Treatment - Initiation Recirc Pumps - Trip Containment Valves - Isolation Core Spray - Start, but no injection Containment Spray - Manual initiation only, following use of EMRVs to control Rx pressure, causing elevated Torus water temperature. The licensee informed the NRC Resident Inspector. Notified R1DO (Krafty).

The following information reported under 10CFR50.72(b)(2)(xi) was provided as an update: Notified the New Jersey D.E.P. of an unplanned plant shutdown which occurred at 0522 07/17/07. There was no environmental impact from the plant shutdown. This notification is required to be made within 12 hours of an unplanned plant shutdown. The licensee informed the NRC Resident Inspector. ENS 4324919 March 2007 22:16:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification Made to New Jersey Department of Environmental ProtectionNotified the New Jersey State, Department of Environmental Protection that Oyster Creek generating station was not in compliance with the NJPDES permit due to equipment malfunction. The malfunction was an electrical bus fault which resulted in a trip of both operating dilution pumps. Alternate power is now being supplied to the dilution pumps. The pumps have been restarted. There has been no observed impact on the environment.. NRC Resident Inspector was notified of this event by the licensee.

Results of the extensive review indicate all sample levels to be within expected ranges for this area. This notification is being made due to heightened public awareness for environmental issues. Samples indicate slightly elevated levels of Cs-137 that are below any reporting criteria. The licensee will notify the New Jersey Department of Environmental Protection. The licensee will notify the NRC Resident Inspector.

The New Jersey Department of Environmental Protection was notified of the sighting of 10 dead and approximately 40 stressed Bluefish in the Oyster Creek discharge canal at 0440 January 29, 2006, following a plant shutdown. The licensee will notify the NRC Resident Inspector.

This is a NRC ENS 24 hr notification per Oyster Creek Environmental Tech Spec Section B 1.1.1 A. At 11:45 on 01/29/06 a total of 100 fish were spotted either dead or stressed in the discharge canal following a Reactor S/D (shutdown) that was completed at 22:22 on 01/28/06" R1DO (N. Perry) notified. The NRC Resident Inspector was notified of this event by the licensee. The state will be notified.

ENS 4215519 November 2005 02:19:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseLost Power to Circulating Water Dilution Pump Elevating Discharge Temperatures to BayAt 2235 the licensee notified the New Jersey Environmental Protection (NJEP) Agency that their circulating water discharge temperature to Barnegat Bay was elevated. This is due to the fact that the licensee lost power to their non-safety related circulating water dilution pump. The licensee is currently working to restore power to the dilution pump and reducing reactor power to minimize the temperature difference between the discharge water and Bay water. The licensee notified the NRC Resident Inspector.

Reactor Scram Due to Generator Trip/Turbine Trip Following Offsite Electical TransientThe licensee reported that an offsite electrical transient on the 230KV transmission line appears to have caused a generator trip and corresponding turbine trip and reactor scram. The scram was uncomplicated. All rods fully inserted. No safety relief valves lifted during the transient. The reactor is stable at 920 psi with decay heat being discharged to the main condenser and fed via the main feed pumps. There was no loss of offsite power during the transient and the plant is in a normal electrical lineup. The specific cause of the generator lockout is still under investigation but is believed to have been caused by maintenance work at the Manitou 230KV substation about 25 miles from the plant. Lightning arrestors were being placed in service at the substation and are believed to have resulted in an electrical disturbance on the grid. The licensee notes that switchgear for the stack lighting did trip during the transient and so the FAA was notified on the stack lighting outage. The licensee plans to remain in hot shutdown for approximately 24 hours before restart. The licensee notified the NRC Resident Inspector.Safety Relief Valve

(EDT) on 9-14-04. The licensee has notified the NRC Resident Inspector.
ENS 4096517 August 2004 17:15:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleaseOffsite Notification - Possible Discharge Permit Violation

At 1145 EDT the Chemistry Tech sampled the Rad Waste Service water overboard line. The sample showed less than 0.02 milligrams per liter (mg/l) residual chloride. The Chemistry Tech adjusted up the chlorination rate. At 1315 a second sample yielded a result of 0.29 mg/l residual chloride. The tech immediately secured the Chloride system, and sampled the intake canal. The results were less than 0.02 mg/l residual chloride. Oyster Creek is monitoring for any environmental impact. The change in residual chloride may not correlate to the increase in chlorination rate, therefore sample error is possible and is under review. The licensee notified the NRC Resident Inspector.

This is a follow-up to an earlier notification made at 15:30 today. Based upon further investigation the Potential NJPES Violation was found to be valid. The residual intake Cl - (Chloride) concentrations were found to be 0.29 mg/l. This is in excess of the 0.2 mg/l limit. The over chlorination event lasted for approximately 2 hours." R1DO (Todd Jackson) notified. The NRC Resident Inspector will be notified of this update by the licensee.

Recent rains pulled out the percolated oil laden water which entered the storm drain. Less than one gallon of oil entered the storm drain. The source of the oil spill (possibly from a vehicle) is under investigation. The licensee notified the NJ Department of Environmental Protection, National Response Center, Environmental Protection Agency and the U.S. NRC Resident Inspector.
ENS 406568 April 2004 16:55:0010 CFR 50.72(b)(2)(xi), Notification to Government Agency or News ReleasePress Release Regarding Law Suite SettlementOyster Creek station is making a press release regarding a settlement with the State of New Jersey related to and environmental event which occurred on September 23, 2002. The press release contains the following information: 'FORKED RIVER, NJ. - The Oyster Creek Generating Station and the State of New Jersey agreed today to settle and resolve all matters related to the Sept. 23, 2002, water permit violation at the station. Under the terms of the agreement reached with the New Jersey Department of Environmental Protection and the Attorney General's office, Oyster Creek will pay a total of $1 million that will be distributed to several state organizations and environmental causes. 'The settlement resolves the 2002 event that resulted in the loss of nearly 6,000 fish. The event occurred because the station equipment was out of service for planned maintenance which caused the discharge water temperatures to rise resulting in the fish kill.' The Licensee notified the NRC Resident Inspector.
