July 26,1999 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PNO-l-99-035 This preliminary notification constitutes EARLY notice of events of POSSIBLE sifaty or public interest significance. The information is as initially received without verification or evaluation, and is basically all that is known by Region I staff in King of Prussia, Pennsylvania on this date.

Notification of Unusual Event Citysrt Cliffs 1 Alert Lusby, Maryland Site Area Emergency Dock;ts: 50-317 General Emergency X Not Applicable Subj:ct: REACTOR SCRAM AND FORCED OUTAGE On July 24, at 2:38 p.m., Calvert Cliffs Unit 1 automatically shutdown from 100 percent power. All control rods fully inserted and all safety (quipment operated as designed during and following the shutdown. The transformer fault protection system initiated a turbine generator trip anc resultant automatic reactor trip (loss of load) due to the failure of th3 gsnerator output transformer (U-25000-12). As of 8:00 a.m. this moming, the unit was in Mode 3 (Hot Standby).

Visibl3 damage to the U-25000-12 transformer "A" phase bushing in the forr1 of black bum marks and perforations were noted. The transformer fira protection deluge system automatically initiated, but no actual fire was observed. Detailed BGE evaluation of the transformer confirmed no cdditional damage was sustained; however, the licensee's investigation into the cause of the failure continues. The licensee plans to replace

- th3 "A" phase bushing prior to Unit i restart.

Upon notification of the automatic shutdown, the resident inspectors responded to the site and monitored the licensee's recovery actions. No problems were noted.

The State of Maryland has been informed. The information in this praiminary notification has been reviewed with the licensee management.
