LOSS OF A PACKAGE CONTAINING 2 MILLICURIES OF 10 DINE-125 (I-125)

On July 13.1999. the licensee contacted the NRC Headquarters Operations Officer to report the loss of a package containing two millicuries of liquid I-125. The material was ordered from the manufacturer. Amersham, on June 4,1999, for an experiment to be conducted on June 14. 1999. The researcher notified the Radiation Safety Office on June 14. 1999 that the package had not been delivered and the Radiation Safety Office staff

-determined that the. package was missing on the same day. The manufacturer informed the licensee that the package containing the radioactive material had been shipped to the licensee by Federal Express on June 4.

1999. The licensee has questioned personnel and examined ) laces of stcrage throughout the facility for the missing package. )ersonnel in the shipping / receiving department. signed a document that a number of packages were received on June 7. 1999, but did not record the number of packages and their Purchase Order Numbers as required by the licensee's internal procedures. Personnel have been retrain d in the pro)er log-in

)rocedures for incoming packages. The licensee is contacting rederal Express to determine if the package was actually delivered.

The District of Columbia has been notified. The licensee has been cortacted and concurs with the technical content of this PN.

The Region 1 Office of Public Affairs is prepared to respond to media incuiries. Region I will conduct an inspection to follow-up on the licensee's actions to recover-the material.
