SUBJECT: NRC INTEGRATED INSPECTION REPORT 50-348/97-05 AND 50-364/97-05.

AND NOTICE OF VIOLATION

Dear Mr. Morey:

On May 10, 1997 the NRC completed an inspection at your Farley Nuclear Plant

(FNP) facility. The enclosed report presents the results of that inspection.

During the six-week period, your conduct of activities at FNP was generally

characterized by safety-conscious operations, sound engineering and

maintenance practices, and careful radiological work controls. However,

certain violations of NRC requirements were identified involving - 1) Failure

to notify the Commission of a change in medical status of an operator: 2)

Several Technical Specifications (lS) surveillance requirements were not being

performed; and 3) Inadecuate measures to assure that special processes (i.e. ,

welding) were controllec in accordance with applicable codes. In light of

past and present examples, we are particularly concerned that your TS

surveillance program did not include a number of required surveillance tests.

Although you identified the missed TS surveillance requirements and discretion

could have been allowed, we decided that this violation warrants citation

because of the multiple examples involved and the extended duration they

existed.

These violations are cited in the enclosed Notice of Violation (Notice), and

the specific circumstances surrounding them are described in detail in the

enclosed report. Please note that you are required to respond to this letter

and should follow the instructions specified in the enclosed Notice when

preparing your response. Your response should also include what additional

actions you have taken to ensure all required TS surveillance tests are

recognized and incorporated in your surveillance test procedures. The NRC ,

will use your response, in part, to determine whether further enforcement

action is necessary to ensure compliance with regulatory requirements.
