DAVIS BESSE ROOT CAUSE FINDING REPORT PLAN 1D On October 30, 1985 we provided Toledo Edison Company with our evaluation of your report related to AFPT overspeed trip throttle valve problems, Plan 10.

The revision of the report which we reviewed indicated that it was Toledo Edison Company's intent to replace the trip spring for the trip and throttle valve on both AFPTs prior to restart of Davis Besse. This recommendation had previously been provided to you in a. letter from Terry Turbine on February 3,

In Revision 3 to Plan 10, submitted on October 31, 1985 (Serial No. 1202),

this commitment has been deleted. It is our position that these springs i should be replaced as per manufacturer's recommendations prior to restart; otherwise an acceptable justification should be provided. The report on Plan ID should be revised to reflect restoration of the comitment or the requested justification for our review. Please provide your revision no later than November 30, 1985.
