APPENDIX A

NOTICE OF VIOLATION

Gulf States Utilities Docket No.: 50-458/85-66

River Bend Station License No.: NPF-50

During an NRC inspection conducted on September 30-October 4, 1985, violations

of NRC requirements were identified. The violations involved failure to follow

procedures concerning Temporary Change Notices (TCNs). In accordance with the

" General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR

Part 2, Appendix C (1985), the violations are listed below: ,

A. Failure to Update Procedures After Issuance of Three TCNs

10 CFR 50, Appendix B, Criterion V, and the licensee's approved quality

assurance program require that activities affecting the quality of

safety-related systems and components be conducted in accordance with

approved instructions, procedures, and drawings.

River Bend Procedure ADM-0003, " Development, Control and Use of

Procedures," Revision 7, dated September 10, 1985, requires that a

procedure change be issued after three outstanding TCNs are approved for

that procedure.

Contrary to the above, eight system operating procedures (SOP-2, -16,18,

-41, -42, -48, -53 and -90) had more than three TCNs, which required a

permanent procedure change, outstanding.

This is a Severity Level IV violation (Supplement I.D)(458/8566-01).

B. Failure to Properly Prepare TCNs

10 CFR 50, Appendix B, Criterion V, and the licensee's approved quality

assurance program require that activities affecting the quality of

safety-related systems and components be conducted in accordance with

approved instructions, procedures and drawings.

River Bend Procedure ADM-0003, " Development, Control and Use of

Procedures," Revision 7, dated September 10, 1985, requi,res that TCNs be

made to the latest approved change to the affected procedure.

Contrary to this, TCNs were issued to SOP-35, -42, -48, and -49 which did

not incorporate previously approved TCNs to those procedures. This

-resulted in two different approved changes'to the same pages.

This is a Severity Level'IV violation (Supplement I.D)(458/8566-02).

C. Change and Issuance of a TCN after Final Approval

10 CFR 50 Appendix B, Critorion V, and the licensee's approved quality

assurance program require that activities affecti,ng the quality of

safety-related systems and components be conducted in accordance with

approved instructions, procedures and drawings.

River Bend Procedure ADM-0003, " Development, Control, and Use of

Procedure," Revision 7, dated September 10, 1985, requires that a

procedure or change be issued as approved.

Contrary to the above, TCN 85-1550 to SOP-0035 was changed and issued

.after final approval of the TCN.

~This is a Severity Level IV violation (Supplement I.D)(458/8566-03).

Pursuant ot the provisions of 10 CFR 2.201, Gulf States Utilities is hereby

required to submit to this office within 30 days of the date of the letter

transmitting this Notice, a written statement or explanation in reply,

including for each violation: (1) the reason for the violations if admitted,

(2) the corrective steps which have been taken and the results achieved,

(3) the corrective steps which will be taken to avoid further violations, and

'(4) the date when full compliance will be achieved. Where good cause is shown,

consideration will be given to extending the response time.
