SUBJECT: NRC INSPECTION REPORT 50-382/96-14 AND NOTICE OF VIOLATION j

Dear Mr. Dugger:

An NRC inspection was conducted December 1,1996 through January 11,1997, at your

Waterford Steam Electric Station, Unit 3 reactor facility. The enclosed report presents the

scope and results of that inspection. ,

During the 6-week period covered by this inspection period, your conduct of activities at l

Waterford was generally characterized by good performance of operations, maintenance, '

engineering, and plant support. However, two violations of NRC requirements were

identified. i

l The first violation involves an inadequate procedure for the calibration of the hydrogen

analyzers. We are concerned that the identified discrepancies indicated a lack of rigor i

during the development of the procedure in tnat a number of the vendor recommended

tests were not being performed. The second violation is of concern because procedures .

were not followed regarding reporting of security lighting deficiencies. In addition,

although not a violation, inspectors had to intervene in a safety-related welding activity to  ;

prevent a procedural violation. We are concerned that these incidents indicate a continuing 1

problem with procedural compliance among your station personnel.

An unresolved item was identified that involves apparently inadequate corrective actions

for water hammer events that occurred in tha low pressure safety injection system on

November 19 and 21,1996. We will conduct additional reviews of this issue in the near

future.

The violations are cited in the enclosed Notice of Violation (Notice) and the circumstances

surrounding the violations are described in detail in the enclosed aport. Please note that

you are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine whether further enforcement action is necessary to ensure compliance with  !

regulatory requirements.
