0 ENCLOSURE 1 NOTICE OF VIOLATION Entergy Operations, Inc. Docket No.: 50-458 River Bend Station License No.: NPF-47 During an NRC inspection conducted on August 26-30 and September 9-13,1996, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," NUREG-1600, the violation is listed below:

10 CFR 50, Appendix B, Criterion 111, requires that design control measures be established to assure that the design bases are correctly translated into specifications, drawings, procedures, and instructions. It also requires that design control measures provide for verifying or checking the adequacy of design, by the performance of design reviews, calculations, or a testing program and that design changes are implemented consistent with the design controls applied to the original design.

Contrary to the above:

1. From March 31,1995 until September 12,1996, the design basis for operation of the standby sers o , vater cooling tower fans was not correctly translated into Alarm Response Procedure ARP-870-55,"P870-55 Alarm Response."
2. As of September 13,1996, the design basis for operation of the standby service water system, which was established in the August 1992 performance of Procedure '

Number TP-92-008," Flow Balance and Design Flow Verification for The Standby Service Water System and the Normal Service Water System," was not correctly translated into System Operating Procedure SOP-0018," Normal Service Water.

3. As of September 13,1996, Calculation 12210-PM-236," Standby Service Water Operation with One Pump Available," dated April 12,1985, which provided the ana,ytical basis for single pump operation of the "A" subsystem of standby service wator following a loss of the Division lli diesel generator, had not been updated to i reflect system modifications and the throttle valve positions established during the l August 1992 performance of Procedure Number TP-92-008," Flow Balance and Design Flow Verification for The Standby Service Water System and the Normal Service Water System."

This is a Severity Level IV violation (458/9627-01)(Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Entergy Operations, Inc. is hereby required to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV,611 Ryan Plaza Drive, Suite 400, Arlington, Texas 76011, and a copy to the NRC Resident inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). 2-violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Your response may reference or include previous docketed correspondence,if the correspondence adequately addresses the required response, if an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.

Where good cause is shown, consideration will be given to extending the response time.
