Gentlemen:

Subject: Inspection No. 50-293/85-26

This refers to the routine safety inspection (50-293/85-26) conducted by Dr. M.

McBride of this office on August 20, 1985 to September 23, 1985 at the Pilgrim

Nuclear Power Station, Plymouth, Massachusetts, of activities authorized by NRC

License No. DPR-35 and to the discussions of our findings held by Dr. M. McBride

with Mr. C. Mathis and other members of your staff periodically during the inspec-

tion and at the conclusion of the inspection. .

Areas examined during this inspection are described in the NRC Region i Inspection

Report which is enclosed with this letter. Within these areas, the inspection

consisted of selective examinations of procedures and representative records, in-

terviews with personnel, and observations by the inspector.

Based on the results of this inspection, it appears that one of your activities

related to approval of licensed reactor operator overtime was not conducted in full

compliance with NRC requirements, as set forth in the Notice of Violation, enclosed

herewith as Append'< A. The violation has been categorized by severity level in

accordance with *.ie revised NRC Enforcement Policy (10 CFR 2, Appendix C) published

in the Federal Register Notice (49 FR 8503) dated March 8, 1984. You are required

to respond to this notice and in preparing your response, you should follow the

instructions in Appendix A.

In addition, we are concerned that corrective actions were not promptly taken in

response to a degraded bl 0h radiation area door lock. Three Radiological Occur-

rence Reports (ROR's) identified this problem over a five-month period. The lack

of response to these ROR's indicates that a weakness exists in your ROR program.

We understand that your staff plans to modify the pro 0 ram, highlighting potentially

significant ROR's that need prompt corrective action. We will review your correc-

tive actions during a future inspection.

The responses directed by this letter and the accompanying Notice are not subject

to the clearance procedures of the Office of Management and Oudget as required by

the Paperwork Reduction Act of 1980, PL 96-511.
