APPENDIX A NOTICE OF VIOLATION Pubite Service Company of Colorado Docket: 50-267/85-14 Fort St. Vrain License: DPR-34 During an NRC inspection conducted on May 1-31, 1985, a violation of NRC requirements was identified. The violation involved failure to follow procedures. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation is Ifsted below:

Failure to Follow Procedures 10 CFR 50, Appendix B, Criterion V, states, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawirigs. " This requirement is implemented by the licensee's. updated Final Safety Analysis Report, Section B.5.2, " Quality Assurance Programs," and Technical Specification 7.4,

" Procedures - Administrative Controls."

1. Fuel Handling Procedure Work Packet FHPWP-100-31, Task 48, Steps 48.1 l and 48.5, required and provided instructions for the proper installation of the second-stage duplex bearings and third-stage duplex bearing slinger washers.

1 Contrary to the above, on May 9,1985, the NRC inspectors determined that, even though the workman had signed off the step as complete and QC had signed off the hold point verifying step completion, the l second-stage duplex bearings had been installed reversed and the wrong slinger washer had been installed on the third-stage causing the gears l to rub.

l This is a Severity Level IV Violation. (Supplement I.D.)

2. Administrative Procedure G-9, " Controlled Work Procedures," Issue 6, I dated December 5, 1984, requires controlled work procedures (CWP) to be
prepared in accordance with the CWP manual.

Controlled Work Procedure Manual, Issue 2, dated January 31, 1985, requires the CWP preparer to use the Checklist of General Planning Considerations and incorporate specific or general housekeeping / clean-liness comments from Attachment 6.11 within the work instructions of the process / inspection / test / report (PITR).

Contrary to the above, on May 31, 1985, the NRC inspector determined 8508060274 e50002

~2-that: (1) housekeeping specific comments had not been incorporated within the PITR for the control room design review modifications; and (2) clean-liness specific comments had not been incorporated within the PITR for the CRD purge line modifications.

This is a Severity level IV Violation. (Supplement I.D.)

Pursuant to the provisions of 10 CFR 2.201, Public Service Company of Colorado is hereby required to submit to this office, within 30 days of the date of this Notice, a written statement or explanation in reply, including for each viola-tion: (1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved; (3) corrective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending your response time. Response to Violation 8514-01 is not required based on apparent adequate corrective actions as indicated in Appendix B to this report.
