10CFR21 Defect 21-93-029: Potential for Motor Pinion Key Failure in g Limitorque Actuators due to Exqsssive Torque Durine Valve Backseating and MOVATS Testing

Dear Sir:

On December 10,1993, Illinois Power notified the NRC Operations Center by facsimile of the subject defect. This initial notification was followed by written notification in letter U-602214 dated December 10,1993. The notification discusses the discovery of a broken motor shaft -

pinion key in the actuator of motor-operated High Pressure Core Spray (IIPCS) system injection and containment isolation valve lE22-F004. Illinois Power evaluated the key failure and concluded the issue was a defect and reportable under the provisions of 10CFR21. The initial and written notifications incorrectly identify the actuator ofvalve IE22-F004 as "model SB, size 3

• with a forty foot-pound,3600 revolutions per minute motor." The description of the actuator should be "model SB, size 3 with an eighty foot-pound,3600 revolutions per minute motor."

Illinois Power re3rets any inconvenience this error may have caused.
