47 CAUSE DESCRIPTION AND CORRECTIVE ACrlONS h lijol]The cause of this event was due to the increased efficiency of the RBCCW Heat Ex- l 8 l changer upon cleaning. Af ter. switching to a cleaned Heat Exchanger, the cooling i iil2i l water was rapidly cooled at an increased heat transfer rate. The increased rate was; Iil3l l unanticipated and caused the drop in the Drywell to Torus AP.
