-~~------**---------~-PROGRAM SCOPE (608) 839-9956 This was a routine, unannounced inspection of a mobile nuclear medicine service that was inspected at Barnes-Jewish West County Hospital in St. Louis, Missouri.

The licensee performed flourine-18 (F-18) labeled FDG studies at this location once per week, averaging 8 patients each week. Doses were based on patient weight and ranged from 8 -24 millicuries per patient. The mobile unit was staffed by 2 certified nuclear medicine technologists (CNMT's).

Unit doses of F-18 were supplied by a local radiopharmacy.

The mobile unit included a patient injection room, quiet room, and control and imaging rooms. The RSO performed periodic radiation safety audits at the unit. PERFORMANCE OBSERVATIONS The inspector observed the CNMT's prepare and administer diagnostic doses of F-18, and noted the use of lab coats, gloves, syringe shields, and personal dosimetry.

A CNMT explained that unit doses were delivered only to the mobile unit, and all radioactive waste was stored in the trailer. Radiopharmacy staff have access to the mobile unit through a key that is stored in a lock box that it attached to the unit. The CNMT's described daily calibration of the dose calibrator, handling and processing of radioactive waste for decay in storage, and daily and weekly surveys for radiation levels and removable contamination.

The inspector reviewed the letter of agreement between the licensee and client, results of surveys of all areas of use before the licensee leaves the client's address, personal dosimetry reports, records of daily dose calibrator constancy checks, survey meter calibration records, annual radiation level surveys in unrestricted areas outside the coach, and records of initial and annual training of the CNMT's. Using a Canberra model RadEyeG gamma survey meter calibrated on 9/12/17, the inspector performed independent surveys outside the mobile unit when the maximum number of patients were in the trailer. Radiation levels were no greater than 1 millirem/hour at 3 feet from the trailer as described in the license. No violations or NRC requirements were identified.
