RAI, Documentation Needed Discussion:
I called Mr. Jensen to discuss his request in letter dated June 12, 2018.
I stated for the two irradiators that were transferred, I needed documentation from the receiving licensee that they received it and the last leak test performed on the irradiator.
I recommended that he update his Statement of Intent as those locations of use no longer need to be on the Statement of Intent.
I stated if he lowers his cesium
-137 activity, then he would need to submit a new Certification of Financial Assurance. He said that he wanted to keep the same activity.
I told him that the manufacturer and model number did not match what was in the sealed source and device registry, so he is going to review was is being obtained.
I discussed that with the new license format that I would appreciate getting and updated list of manufacturer, model number of irradiators along with the manufacturer and model number of the sources in the irradiators, and how much material he would like to be licensed for each model irradiator.
Action Required:
I sent a table to help Mr. Jensen obtain what I wanted from each irradiator.
