Dear Ms. Stillwell,
In order to consider Dr. Kenamond as the Radiation Safety Officer, we will need the following additional information:
(1) Documentation that he has had training in the radiation safety, regulatory issues, and emergency procedures for 10 CFR 35.400 and 35.600 materials.
This training requirement may be satisfied by completing training that is supervised by a Radiation Safety Officer, authorized medical physicist, or authorized user, as appropriate, who is authorized for 35.400 and 35.600 materials.
(2) Written attestation, signed by a preceptor Radiation Safety Officer (Thomas Neils, M.D.), stating that Dr. Kenamond has satisfactorily completed training in the radiation safety, regulatory issues, and emergency procedures for 10 CFR 35.400 and 35.600 materials, and has achieved a level of radiation safety knowledge sufficient to function independently as a Radiation Safety Officer.
Once the above described training is complete, document it on an NRC Form 313A(RSO).
Please complete the following sections:
(1) Part I, Table 3.c., (2) Check box 4 of Part I, (3) Part II, First Section, Number 3 for authorized user, and (4) Part II, Second, Third, and Fourth Sections.
