"A valve plug originally supplied as a spare parts order was returned to Fisher after the customer discovered a machining error was present on the plug. The Type 461 valve plug has a groove machined across the seating surface of the plug. This groove is required to provide a controlled leak rate thru the valve when the plug is on the seat. The issue recently identified shows that the machined groove did not extend through the full length of the seat. This could lead to a situation in which the intended/required leakage for this design is not fully achieved."

Affected plants are: Waterford-3 and St Lucie.

The equipment affected by this FIN notice are replacement Valve Plug/Stem Assemblies for Type 461 valves supplied under the following machining drawing numbers: 25A0413, 29A7841 or 27B5132. Fisher records indicate two sites were supplied the potentially affected assemblies.

Purpose:

The purpose of this Fisher Information Notice(FIN) is to alert Entergy and Florida Power and Light that as of 6 June 2014, Fisher Controls International LLC ("Fisher')

became aware of the possibility of a situation which may affect the performance of the applicable equipment.

Fisher is informing the NRC 'of this circumstance in accordance with Section 21.21(b) of 10CFR Part 21.

This notice applies only to the subject equipment supplied by Fisher as identified above.

Discussion:

The Type 461 valve plug has a groove machined across the seating surface of the plug. This groove is required to provide a controlled leak rate thru the valve when the plug is on the seat.

The issue recently identified shows that the machined groove did not extend through the full length of the seat. This could lead to a situation in which the intended I required leakage for this design is not fully achieved.

Action Required: Evaluate any affected Type 461 valve plugs in inventory to ensure the machined groove extends across the plug seating area.

Fisher is revising the affected drawings for increased clarity and implementing appropriate corrective actions per CAR 1727.
