Matt, It was a pleasure to meet you and Will last week and to speak with Mary. During the meeting, you had asked for additional information regarding the failed coupling at Palisades. In reviewing the information for you, I realize I incorrectly relayed that a previous failure had occurred in the late 1990s; the previous failure was in 2009.

You had also asked for information about the coupling, its location in relation to the rest of the plant, and how the failed coupling had been discovered. The pipes connected by the faulty coupling are well outside of the reactor building, in the water intake area near the shore. There are three of these intake pipes in total; safety regulations require at least one to be operable in case of an emergency. The plant was operating under normal conditions when indicators in the control room showed a change in the flow of water from one of the three service pumps, the licensee then traced the problem to the coupling. The problem has been corrected and is not affecting plant operations.

I will keep you informed when the Special Inspection Team releases an inspection report and any findings.
