SUBJECT:

Final Reactor Operator (RO) and Senior Reactor Operator (SRO) License Applications As required by the reference 1, the Omaha Public Power District (OPPD) is providing the final Form NRC-398, "Personal Qualification Statement," for the candidates participating in the December 10, 2015, Reactor Operator/Senior Reactor Operator (RO/SRO) license exams at Fort Calhoun Station (FCS). These applications are for three (3) RO candidates, two (2) SRO upgrade candidates and four (4) direct SRO candidates.

Due to the confidential nature, the enclosures will only be provided for the copy being sent to the Operations Section Branch Chief in Region IV.
