UPDATED INSPECTION PLAN FOR GRAND GULF NUCLEAR STATION, REPORT 05000416/2019005

Dear Mr. Larson:

The enclosed inspection plan lists the inspections scheduled through June 30, 2021, for Grand Gulf Nuclear Station. The U.S. Nuclear Regulatory Commission (NRC) provides the inspection plan to allow for the resolution of any scheduling conflicts and personnel availability issues.

Routine inspections performed by resident inspectors are not included in the inspection plan.

You should be aware that the agency is pursuing potential changes to the Reactor Oversight Process (ROP) including changes to engineering inspections (SECY-18-0113, Recommendations for Modifying the Reactor Oversight Process Engineering Inspections), and other changes to the baseline inspection program described in SECY-19-0067, Recommendations for Enhancing the Reactor Oversight Process. Should these changes to the ROP be implemented, the engineering and other region-based inspections are subject to change in scope, as well as schedule, beginning in January 2020. The inspections listed during the last 12 months of the inspection plan are tentative and may be revised. The NRC will contact you as soon as possible to discuss changes to the inspection plan should circumstances warrant any changes.

As discussed in the 2018 annual assessment letter dated March 4, 2019 (ADAMS Accession No. ML19064A731), the NRC determined the performance at Grand Gulf Nuclear Station to be in the Regulatory Response Column (Column 2) of the Reactor Oversight Process Action Matrix beginning the third quarter of 2018. Therefore, the NRC plans to conduct a supplemental inspection in accordance with Inspection Procedure 95001, Supplemental Inspection Response to Action Matrix Column 2 Inputs. The objectives of the supplemental inspection are: (1) to assure that the root causes and contributing causes of significant performance issues are understood, (2) to independently assess and assure that the extent of condition and extent of cause of significant performance issues are identified, (3) to assure that corrective actions taken to address and preclude repetition of significant performance issues are prompt and effective, and (4) to assure that corrective plans direct prompt actions to effectively address and preclude repetition of significant performance issues. This inspection commenced during the week of August 26, 2019. From December 9, 2016, through August 14, 2019, the NRC issued 11 Severity Level IV traditional enforcement violations associated with impeding the regulatory process. In April of 2019, the NRC conducted an inspection to assess your evaluations and corrective actions associated with these issues using Inspection Procedure 92723, Follow Up Inspection for Three or More Severity Level IV Traditional Enforcement Violations in the Same Area in a 12-Month Period. Based on the results of this inspection that are documented in NRC Integrated Inspection Report 05000416/2019002 (ADAMS Accession No. ML19226A236) the NRC was not able to conclude that the actions taken were adequate to meet the inspection objectives.

Therefore, the NRC plans to perform an additional inspection that will be scheduled after you notify the NRC of your readiness.

On March 12, 2018, the NRC issued a Confirmatory Order (ADAMS Accession No.

ML18072A191) (EA-17-132 and EA-17-153) to the Entergy fleet that included Grand Gulf Nuclear Station. The NRC plans to conduct a Confirmatory Order follow-up inspection at Entergy Corporate Headquarters in Jackson, MS, of the Confirmatory Order actions required to be completed by the end of calendar year 2019, in accordance with Inspection Procedure 92702, Followup on Traditional Enforcement Actions Including Violations, Deviations, Confirmatory Action Letters, Confirmatory Orders, And Alternative Dispute Resolution Confirmatory Orders. This inspection will be conducted during the first quarter of 2020 on a date to be determined by the NRC and Entergy.

In the 2018 annual assessment letter, the NRC identified a cross-cutting theme in the Human Performance area. Specifically, 20 findings had a cross-cutting aspect in this area during the 2018 end-of-cycle assessment period. Because this theme criterion is no longer met as of the assessment period ending on June 30, 2019, the NRC has determined that a cross-cutting theme in the Human Performance area no longer exists.
