On December 31, 2019, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Point Beach Nuclear Plant and discussed the results of this inspection with Mr. B. Woyak, Site Safety Assurance and Learning Director, and other members of your staff.

The results of this inspection are documented in the enclosed report.

One finding of very low safety significance (Green) is documented in this report. This finding involved a violation of NRC requirements. We are treating this violation as a non-cited violation (NCV) consistent with Section 2.3.2 of the Enforcement Policy.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Point Beach Nuclear Plant, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. List of Findings and Violations Main Feedwater Isolation Valve Component Materials not Suitable for Environmental Conditions Cornerstone Significance Cross-Cutting Report Aspect Section Mitigating Green None (NPP) 71111.15 Systems NCV 05000266,05000301/2019004-01 Open/Closed The inspectors identified a Green finding and associated Non-cited Violation (NCV) of Title 10 CFR Part 50, Appendix B, Criterion III, "Design Control," when the licensee failed to assure the main feedwater isolation valve actuators and supporting subsystem components installed in the plant were suitable for the environment under which they were required to operate.

PLANT STATUS

Unit 1 began the inspection period at rated thermal power. On October 23, 2019, power was reduced to below 60 percent to support turbine stop and governor valve testing. The Unit returned to rated thermal power on October 24, 2019, and remained at or near rated thermal power for the remainder of the inspection period.

Unit 2 operated at or near rated thermal power for the entire inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed plant status activities described in IMC 2515, Appendix D, Plant Status, and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Remedial Training and Re-examinations The inspectors evaluated the effectiveness of remedial training conducted by the licensee, and reviewed the adequacy of re-examinations for licensed operators who did not pass a required requalification examination.

Operator License Conditions The inspectors evaluated the licensees program for ensuring that licensed operators meet the conditions of their licenses.

Control Room Simulator The inspectors evaluated the adequacy of the facility licensees control room simulator in modeling the actual plant, and for meeting the requirements contained in 10 CFR 55.46.

Problem Identification and Resolution The inspectors evaluated the licensees ability to identify and resolve problems associated with licensed operator performance.

(1) Maintenance activities related to the failure of Unit 2 feed water isolation system in January of 2019, and quality control issues related to the commercial grade dedication of reactor protection replacement relays, which failed in April and May of 2019.

(1) The inspectors reviewed the licensees corrective action program, trend reports, major equipment problem lists, system health reports, self-assessments, and additional documents, to identify trends that might indicate the existence of a more significant safety issue.

INSPECTION RESULTS

Main Feedwater Isolation Valve Component Materials not Suitable for Environmental Conditions Cornerstone Significance Cross-Cutting Report Aspect Section Mitigating Green None (NPP) 71111.15 Systems NCV 05000266,05000301/2019004-01 Open/Closed The inspectors identified a Green finding and associated Non-cited Violation (NCV) of Title 10 CFR Part 50, Appendix B, Criterion III, "Design Control," when the licensee failed to assure the main feedwater isolation valve actuators and supporting subsystem components installed in the plant were suitable for the environment under which they were required to operate.

Description:

On January 30, 2019, the licensee received a Main Feedwater Isolation Valve (MFIV) trouble annunciator. The licensee dispatched an operator to investigate and found the Unit 1 main feedwater isolation valve (MFIV) actuator extend 3-way valve, 1CS-256B, blowing air to the atmosphere. The operator also reported that the pressure at the MFIV accumulator, 1CS-256B, was reading 200 psig and lowering. The licensee declared Unit 1 Train B MFIV, 1CS-3125, inoperable and shut the associated air valve to stop the air leak. This allowed the accumulator to restore pressure above the minimum Technical Specification requirement of 227.5 psig. The MFIVs are safety-related and required to isolate the feedwater system in the event of a main steam line break inside containment.

The licensee determined that the cause of the valve leakage was "extreme" cold temperatures. At the time, the local area temperature was 3 degrees F. The licensee determined that the o-ring seals and valve seats (elastomers) were made from a rubber compound that lost its flexibility at that low temperature. In addition, the elastomers would shrink at a much faster rate than the surrounding metal challenging their ability to seal properly. The licensee added portable heaters to the area, which raised the local area temperature. The licensee unisolated the air to the valve, verified that valve was no longer leaking, and returned the components and system to operable within the technical specifications allowed outage time of 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />.

A follow-up by the resident inspectors questioned why the valve leaked at a temperature of 3 degrees F when they were qualified for -20 degrees F. The licensee determined that the

'Viton' elastomers included in the components was not effective at sealing in significantly cold temperatures. The licensee also found the valves they installed were only manufactured for a minimum temperature +20 degrees F, when the design called for components to be qualified down to -20 degrees F. This was due to some communication and verification errors between the licensee and vendor supplying the components. The extent of condition identified 24 valves that could directly affect the performance of the MFIVs that were installed without meeting design specifications. The licensee also identified more than 96 components that utilize an elastomer that could be adversely affected by the cold weather. The MFIVs and their associated components were installed in 2011 under Engineering Change (EC) 258382 for Unit 1 and EC 258480 for Unit 2.

Corrective Actions: The licensee added portable heaters to the area to raise local area temperatures and revised their cold weather procedure to ensure the area remains above 35 degrees F throughout the cold season. The licensee is planning on adding heat tracing to the components of concern as well as replacing the components with an appropriate material that will meet the original design criteria.

Performance Assessment:

Performance Deficiency: The inspectors determined that failure to ensure systems and components were capable to perform their design function in the associated environmental conditions was a performance deficiency. Specifically, the licensee installed safety-related components with elastomers in an environment that could fall below the minimum temperature for its application, which caused the valve to leak and render the system inoperable.

Screening: The inspectors determined the performance deficiency was more than minor because it was associated with the Equipment Performance attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the MFIV actuator extend 3-way valve, 1CS-256B, began leaking which depressurized the safety-related accumulator below the minimum operable pressure of 227.5 psig. This rendered the associated MFIV inoperable because it can no longer be assured to close during a main steam line break event.

Significance: The inspectors assessed the significance of the finding using Appendix A, The Significance Determination Process (SDP) for Findings At-Power. Specifically, the inspectors determined that this finding is of very low safety significance (Green) because they answered "No" to all questions in Exhibit 2.

Cross-Cutting Aspect: Not Present Performance. No cross-cutting aspect was assigned to this finding because the inspectors determined the finding did not reflect present licensee performance.

Enforcement:

Violation: Title 10 CFR Part 50, Appendix B, Criterion III, Design Control, requires, in part, that measures be established for the selection and review for suitability of application of materials, parts, equipment, and processes that are essential to the safety-related functions of the structures, systems and components.

Contrary to the above, since June 7, 2011 (for Unit 2), and December 9, 2011 (for Unit 1), the licensee failed to review for suitability of application of materials essential to the safety-related functions of the MFIVs. Specifically, the licensee utilized a type of elastomers that were not appropriate for the environmental conditions (extreme cold) they could be subject to.

Enforcement Action: This violation is being treated as an non-cited violation, consistent with Section 2.3.2 of the Enforcement Policy.

Observation: Semiannual Trend Review 71152 During the inspection quarter, the inspectors reviewed a significant number of licensee corrective action program documents to assess the following performance attributes:

• complete, accurate, and timely documentation of the identified problem in the corrective action program;

• evaluation and timely disposition of operability and reportability issues;

• consideration of extent of condition and cause, generic implications, common cause, and previous occurrences;

• completion of corrective actions in a timely manner commensurate with the safety significance of the issue; and

• identification of negative trends associated with equipment performance that can potentially impact nuclear safety.

The inspectors reviewed the site's response to several deficiencies associated with control rods due to the increasing number and repetitive nature of the deficiencies. The inspectors identified that in 2019, the rod control and position indication systems experienced several occurrences of either blowing fuses during rod movement or experiencing erratic indication while stationary.

The inspectors noted that the failure of rod control fuses in August 2019 was identical to a failure that occurred in December 2018. In December 2018, the failure caused Unit 2 Rod F-6 to insert 20 steps into the core. Troubleshooting efforts were not able to identify any direct causes, so the licensee implemented a modification to change the fuse rating from 10 Amps to 15 Amps to account for small power spiking. In August 2019, a similar failure caused Rod F-6 to insert completely into the core. Again, troubleshooting efforts were unable to identify the cause, so the licensee implemented another modification to change the fuse rating from 15 Amps to 25 Amps. Additionally, the licensee changed the frequency of rod testing to minimize the number of times the rods needed to be moved, therefore limiting the chances for the fuses to fail. The licensee also recognized this same rod experienced a similar failure in December 2010 with no cause identified. The licensee is planning additional troubleshooting and repairs during the upcoming outage. The inspectors independently reviewed the licensees efforts and verified the rods remain capable of performing their safety-related function.

The inspectors also noted an increasing frequency of erratic control rod indications. In November 2019, Unit 2 Rod J-4 began intermittently indicating positions other than its actual 228 steps. Troubleshooting did not identify any direct causes and the licensee was unable to correct the erratic indications. The licensee declared J-4 rod position indication inoperable and continues to perform the required TS actions. In December 2019, Unit 1 Rod H-8 began experiencing the same issue. Troubleshooting did not identify a direct cause and the licensee was unable to correct the erratic indication. The licensee declared H-8 rod position indication inoperable and continues to perform the required TS actions. The licensee is planning additional troubleshooting and repairs during the upcoming outages.

The inspectors did not identify any findings or violations of more than minor significance.
