Haddam Neck Plant Response to I&E Inspection No. 50-213/86-19 Pursuant to the provisions of Section 2.201 (" Notice of Violation") and Appendix C (Enforcement Policy) of the NRC's Rules of Practice (10CFR2), this report is submitted in reply to Reference (1), which informed Connecticut Yankee Atomic Power Company (CYAPCO) of a violation, identified during a routine inspection at the Haddam Neck Plant from July 7-11, 198 Alleged Violation:

10 CFR 50; Appendix B, Criterion V, Technical Specification (T.S.) 6.8 and the Northeast Utilities Quality Assurance Program require activities affecting quality to be prescribed and accomplished in accordance with documented procedures. The following licensee procedures prescribe quality affecting activities:

Administrative Control Procedure 1.2 - 5.1, Trouble Reporting System and Automated Work Orders (AWOs), requires work activities to be controlled and documented, including (1) the implementation of a supplemental work order when the work scope increases and (2) the correct processing of Material Issue Lists (MILS). Procedure 1.2 - 2.5, Classifyino and Upgrading Spare Parts, requires MILS to be correctly classified and requires an appropriate revie! be performed to determine if a spare part is a Category 1 ite Contrary to the above, as of July 11, 1986, AWO CY-86-03247 was not controlled and documented in accordance with ACP 1.2 - 5.1 in that, (1) a supplemental work order was not generated when the work scope changed from relugging one termination to relugging all terminations within CH-MOV-298, and (2) the associated MIL was not correctly classified as Category I for the spare parts (lugs) utilized to complete the wor This is a Severity Level IV violation (Supplement I).

Response:

As an immediate corrective action, the failure to follow procedure was discussed with the job supervisor. He acknowledged i the error As a planned corrective action, the requirement to follow procedures will be emphasized with all employees in department meetings planned for September / October. Additionally, at these meetings, all employees will be reminded of the plant disciplin-ary action policy regarding failure to follow procedures. The initial action of this progressive discipline sequence, consis-ting of a verbal warning, was administered in this case.
