4.1 Exemption Request By a letter dated March 12, 1986, the licensee submitted to NRR a request for exemption from several Appendix J leak test requirements.

The proposed change request was divided into two parts: the first part consisted of permanent exemptions related to types "A" and "C" tests, and the second part was schedular exemptions related to type
"C" leak testing only. However, the second part of the proposed schedular exemptions could impact the type "A" test should the pro-
- posed penetration or penetrations be isolated during the type "A" test.

The items under review by NRR under the proposed exemption request were not reviewed during this inspection, pending final NRR resolu-tion. They included the following:

Allow water tests on the penetrations: P-3, 7, 10, 11, 24, 28, 30, 34, 38, 60, 61, 63, 66, 67, 68, 69, 74, 75, 76, and 7 .2 Procedure Review The CILRT test procedure, SUR 5.7-108, Revision 2, was reviewed for test adequacy and system isolation. Procedural step 9.1 identified six penetrations which were isolated during the type "A" ILR Their Local Leak Rate Test (LLRT) results would be added to the results of the ILRT. They were penetrations P-28, 29, 63, 70, 80, and 8 The inspector identified two additional penetrations which were programmatically isolated in the test procedur They were penetra-tions P-12B and P-13, and the licensee acknowledged that those two penetrations would be added in procedural step 9.1 and their LLRT results would be added to the ILRT result .3 Test Instrumentation The inspectors reviewed the calibration records for the sensors used for the CILRT. It was ascertained that the instruments were cali-brated within the 6 month period prior to the test and that the standards used were traceable to the National Bureau of Standard This is required per industry standard ANSI /ANS 56.8-1981. The inspectors also verified that the instrument system satisfied the instrument selection guide of ANSI /ANS 56.8-1981. The inspectors observed the operation of the automatic data collection system and

witnessed the arrangement of the RTD's and dew cells in the contain-ment. All instruments observed were placed in areas which would permit a good average of the containment bulk drybulb and dewpoint temperatur No unacceptable conditions were identifie .4 FAILURE OF "AS FOUND" CILRT At 2300 hours, Wednesday, April 2, 1986, the containment pressuri-zation was commenced after replacing the leaky gasket for the flanges connecting the air compressors to the containment pressurization penetration. At 0625, April 3, 1986, the compressors were secured and the containment stabilization period was starte In accordance with procedural requirements, containment temperatures and pressure were monitored every 15 minutes. The hardwired computer calculated the containment air mass, Least-Square-Fitted (LSF) leak rate and the upper confidence level continuousl In order to verify the validity of the computer software for the ILRT calculations, the inspector made an independent calculation as follows:
Data Point at 1215 hours, April 3, 1986 Average RTD Readings = 72.3735 F Vapor Pressure (Dewcells) = 0.1887 psi Containment Air Pressure = 30.6087 psia Containment Volume = 2,232,000 cubic feet The inspector calculation of containment air mass with a hand-held calculator was 344,443.51 lb. and was 0.6% smaller than the computer
, calculation. The inspector determined that the above difference was due to the truncation errors from the hand-held calculato Based on the data points of the first 6 hours (0700 hour to 1300 hour, April 3,1986) a straight line correlation of the containment mass points indicated a leak rate of 0.475%/ day, exceeding the maxi-mum allowable leak rate of 0.11071s/ day. Subsequently, the licensee acknowledged that the "AS FOUND" CILRT failed. The following is a summary of the stabilization failures as given in the official CILRT log book:

Based on the data points, the "As Left" CILRT appears to meet the acceptance criteria. Final determination is pending review of the summary technical report, which will include the LLRT results for the isolated penetration .3 Instrument Verification Test The instrument verification test was performed after the 24 hour leak rate test cy superimposing a known leakage of 3.62 SCFM as measured by the bypass flowmeter. the total leakage which includes the superimposed leak was measured from the slope. Estimated results are as follows:
Total Leakage = 0.0938 %/ day Superimposed Leakage = 3.62 SCFM = 0.1147 %/ day Therefore, CILRT = 0.0938 - 0.1147 = -0.0209 %/ day Since, 0.25 L = (0.25) x (0.1102) = 0.02755 %/ day tm l-0.0209 % day [ < 0.25 L =0.02755 %/ day t
The result is acceptabl .0 CILRT Personnel Training and Qualifications The Qualifications Records of Test personnel were reviewed, and test personnel interviewed. Their qualifications met the requirements speci-fied in ANSI N 18.1-1971 " Selection and training of nuclear power plant personnel". They were knowledgeable of their responsibilities and tech-nical aspects of leak testin No unacceptable conditions were identifie .0 Tours The inspectors made several tours of the plant including the reactor building, primary auxiliary building, control room and plant exterior (containment, compressor, and CILRT data acquisition areas) in order to monitor the activities related to the preoperation for and performance of the CILRT. The inspectors noted no unusual or unacceptable condition .0 QA/QC Coverage During the preparation and initiation of the CILRT the inspectors verified QA/QC involvement. QA/QC was present during final containment walkdown and pretest briefing by the CILRT director.
