'An initial' audit of the requalification program was conducted on August 6 - 10,-1984, that indicated enough significant concerns to warrant a more indepth review of the requalification program at the Haddam Neck

' facility. This NRC audit was conducted on November 14 - 16, 1984. Results from this audit were presented at a management meeting held on December 17, 1984. At this meeting the following commitments were <

obtained from the licensee: 1) submit a written proposal to NRC Region I '

by January 31, 1985, a short term plan for an augmented requalification 1 program for training individuals whose licenses are to be renewed before 1 1986; 2) a plan for' ongoing self-initiated monitoring / auditing of re-qualification program. effectiveness, and a long term requalification

training program to be implemented in 1985. A second management meeting was held in Region I on March 25, 1985, to discuss the scope and content of the Licensed Operator Requalification Training Program submitted by the licensee as a result of the meeting held on December 17, 198 Further commitments were obtained from this meeting on March 25 regarding operating license renewals, i.e. to provide: 1) documentation for those individuals that will be exempted from the theory upgrade portion of the program; 2) details of an independent third party evaluatirA:; and, 3)

names of those individuals who will not complete the short term program with appropriate justification for license renewa . 3.0 Scope of Inspectioi This inspection assessed conformance to the commitments of the December 17, 1984 and March 25, 1985 management meetings. I,t included review of the proposed schedule and implementation dates for the Licensed Operator Continuing Training Program for 1986, results of the third party evaluation and audits, direct observation of requalification training in progress including review of written examinations and observation eflcral examination .1 Implementation of the Short-Term Training Program The licensee documented in his February 1, 1985 response to the NRC those interim actions that would be implemented for an augmented upgrade training program for individuals whose licenses are to be renewed before 1986. This constituted approximately 22 operators to

/ receive this program. The licensee conducted and completed a train-sing session for six operators in May 198 Waivers were requested for five additional operators in correspon-dence received from the licensee on July 19, 1985. Following a review of the submitted justification, Region I granted the waivers for those individuals on July 30, 198 Three of the remaining individuals were given a special examination designed and administered by a third party on September 6, 198 All three operators achieved a passing grade of greater than 80%.

4.0 Implementation of the Long Term Program 4.1 An independent third party evaluation of the requalification program was conducted on May 3, 6, 13 and 22, 1985. The results of these audits completed on May 28, 1985 were reviewed by i1RC Region I on August 10, 1985. Findings from the NRC review concurred with the independent-third party evaluation in that the Connecticut Yankee station license requalification program is adequate in scope and content such that NRC regulations, policies and guidelines are being met at this time, however, the proposed 1986 licensed operator con-tinuing training program comprising a rotating one-in-six week con-tinuing process, utilizing the CY simulator appears to be a more effective means of conducting an adequate requalification progra .2 In addition three requalification classroom sessions were conducted on May 13 - 24, June 3 - 14, and September 9 - 20, 1985.
