The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector. The inspection findings are as follows:

[lJ Temporary Job Site Inspection IU Methodist Hospital, Indianapolis, IN PROGRAM SCOPE This was a limited-scope, reciprocity inspection of a service provider licensed by the State of California. The licensee provided preventive maintenance, installation, and dismantlement/removal of various HOR and gamma stereotactic radiosurgery (GSR) devices. The licensee authorized 11 individuals to perform these services. Alpha Omega Services, Inc. filed its reciprocity request, NRC Form-241, on 5/7/2018 for the service activities scheduled during the 2018 calendar year. The licensee conducted services for a customer, IUPUI/ IU Methodist Hospital under reciprocity tracking system number 156117. The customer received the source cask on 12/10/2018. The licensee installed the sources into the GSR unit on 12/14/2018. On 12/11-12/2018, the licensee removed and packaged the old sources from the customer's GSR unit. Licensee personnel performed the package loading and transportation prep activities on 12/14/2018. All licensed activities were performed by three authorized source handlers. The source handlers wore their assigned whole body Luxel dosimeters; left and right extremity ring badges (exchanged quarterly); and electronic dosimeters (read daily). The YTO 2018 maximum exposures for the source handling personnel were reported as 209 mrem TEOE and 242 mrem SO This inspection consisted of interviews with selected licensee personnel; a review of select records; and observations of licensee staff performing the service activities described above the customer's facility. The inspector observed the security of byproduct material, personnel exposure controls, use of personnel monitoring, postings, source leak tests, and area wipe tests and surveys. Alpha Omega Services staff demonstrated their knowledge in the licensee's operating and emergency procedures, site specific tasks, stopping points, and their responsibilities during the source removal/

installation tasks.
