The inspection consisted of selective examinations of procedures and representative records, interviews with personnel, and observations by the inspector. The inspection findings are as follows:

[Z] Temporary Job Site Inspection 27566 US-12, Edwardsburg, Michigan PROGRAM SCOPE This was a limited-scope, reciprocity inspection of a sealed source recovery performed by an environmental services group licensed by the Commonwealth of Kentucky. The licensee was contracted by a company who discovered that a sealed source, containing approximately 4.5 mCi of cesium-137 (Cs-137), broke free from its weld on the source rod of a Troxler Model 3430 portable gauge while taking a measurement at a temporary job site in Edwardsburg, Michigan. The best estimated location of the source was 2 feet underground beneath four inches of a recently poured sidewalk. The licensee filed its reciprocity request with the NRC for this service on May 24, 2019. All activities were performed by two authorized individuals employed by the license This inspectors observed the licensee perform radiological surveys of the sidewalk to confirm the best estimate of the source location. Once this location was determined, licensee personnel cordoned off the area and posted it as a radiologically controlled area until successful recovery of the source was achieved and contamination surveys were completed. The inspectors observed a safety briefing given by the licensee that discussed the potential hazards on the work site. The licensee staff wore personal dosimetry and provided direct reading dosimeters to three individuals that were contracted to remove the concrete above the source. The inspectors then observed excavation of the concrete, periodic surveys and wipe tests, excavation of the soil, and location of the source with no issues noted. Once the source was located, the inspectors observed the licensee use long handled tools to perform a wipe test for leakage and place the source into a shielded container. After the source was removed from the ground, the licensee performed surveys of the surrounding soil and concrete to determine that there was no indication of residual contamination. All individuals that entered the controlled area were also surveyed for contamination. The inspectors performed independent and confirmatory surveys to verify these results. Following the surveys, the area was released from radiological contro The licensee stated that the source would be transported to a waste broker in Wampum, PA. The inspectors observed the licensee package the source within a Type A container, determine the transport index, block and brace the container, and generate the shipping papers with no issues noted. Dosimetry results indicated no exposures in excess ofNRC limit No violations ofNRC requirements were identified during this inspection.
